About the Department

Kent County Community Action is seeking a detail-oriented Community Resources Specialist to support the Senior Services Program. This role involves working directly with predominantly English-speaking and Spanish-speaking older adults to assess needs, determine eligibility for services, and connect individuals and families to appropriate resources.

Key Responsibilities:

  • Conduct professional assessments to determine service needs and eligibility for County and community resources.
  • Create client-centered action plans and provide counseling and guidance on available health, mental health, financial, and community resources.
  • Advocate for vulnerable populations and support older adults by scheduling, coordinating, and attending appointments when needed.
  • Refer clients to County programs and community resources; assist with applications, benefits enrollment, and service coordination.
  • Perform home visits and wellness checks for homebound, isolated, or physically compromised older adults.
  • Maintain accurate case records and monitor client participation and progress. 
  • Apply principles and techniques of social work case management in client interactions.
  • Ensure compliance with federal and state regulations regarding data privacy and the collection and verification of client information.
  • Maintain positive, respectful communication with clients from diverse cultural, socio-economic, and educational backgrounds.
This position operates under general supervision and requires the ability to exercise independent judgment and discretion in daily responsibilities.

Minimum Qualifications

Bachelor’s degree from an accredited institution in sociology, psychology, behavioral sciences, human services, or a related field, and a minimum of two years’ experience in case work or project management are required for this position. Equivalent combinations of education and experience may be considered.

Valid Michigan driver’s license required.

Bilingual skills strongly preferred.
