Position Summary:
The Care Coordinator will work in support of the care team through collaboration with other team members to provide seamless treatment management for clients enrolled in outpatient services.
Primary Functions:
1. Facilitate appointments for Just In Time scheduling
2. Coordinates care within Swope Health Services, pharmacies, and other providers in the community
3. Maintains communication between care team providers involved with client care
4. Provides linkages to community resources for new and established patients
5. Provides assistance with Mental Health Liaison data entry requirements
6. Conducts record reviews for presence of pertinent funder driven documentation
7. Relays relevant information to team members across systems (i.e. upcoming appointments, multiple same day appointments, needed case management supports, hospital discharges, etc.)
8. Encourages and promotes the use of Health Information Technology resources
9. Documents within EMR in a manner most beneficial to the care team for wraparound support
10. Coordinates with the care team for appropriate referrals for specialty care
11. Completes all reports in a timely manner
12. Uses protocols and policies and procedures to guide care
13. Completes audits for quality management
14. Tracks that all required Primary, Behavioral, and Alcohol and Substance Abuse assessments and screenings for each client are completed and/or updated in a timely manner
15. Assists in discharge planning of patients across programs
16. Attends all required training and continuing education as required or provided.
17. Other duties and/or responsibilities as assigned
Educational Requirements:
BA, BS Degree or Higher
Experience & Qualifications:
1. BA, BS Degree or Higher
2. 3 to 5 years related experience preferred
3. Information technology software experience
4. Strong written and verbal communication skills
5. Internal and External Customer Service orientated
6. Professional telephone etiquette
7. Exhibits the ability to work effectively with people (staff, visitors and clients).
Certification & Licensure Requirements
1. Completion of required forms per guidelines or protocols
2. Ability to work independently, and manage multiple responsibilities appropriately
3. Ability to document in a concise and professional manner
