%3Cp style=%22font-family: Arial;%22%3EThe position is part time with 0.2 FTE for Family Practitioner for Outpatient Emerging Adult Mental Health Treatment Clinic (EATC).%3Cbr%3ENumber of positions available: 1%3Cbr%3ELocation: RRDTC Outpatient Clinic%3Cbr%3E- Perform duties related to patient care (e.g., attend multidisciplinary case management rounds as required, complete new assessments and as needed follow up meetings with patients and their families and/or case coordinators/ family counsellors, engage in discussions with case managers, for a minimum of 1 full day/week to be further allocated as per the requirements and mutual discussion between the physician and the division chief.%3Cbr%3E- Work with the physician lead of the program or case managers to help patient achieve stability and functional goals.%3Cbr%3E- Provide timely documentation of clinical encounters and any other necessary documentation.%3Cbr%3E- Support colleagues in providing cross coverage for vacations negotiable for frequency.%26nbsp;%3Cbr%3ESpecific goals%26nbsp;%3Cbr%3E- Assessments and follow up of patients admitted or accepted to the wait list for the EATC. 10-20% of incoming cases do not already have a primary care physician.%26nbsp;%3Cbr%3E- Ensuring the completion of documentation on time.%26nbsp;%3Cbr%3E-Coordinating care for the complex medical needs some of our population experience, including liaising with other services (ex. Neurology, Hematology, Rheumatology, etc.)%3Cbr%3EEATC works with youth aged 16-24 who present with a series of complex mental health needs, including both %26ldquo;on-ramp%26rdquo; and %26ldquo;off-ramp%26rdquo; support in a longitudinal fashion for up to a year of care. Many of these youth also present with complex medical needs.%3C/p%3E

Qualifications:

Eligible candidates will have an MD and must be eligible for certification with the Royal College of Physicians & Surgeons of Canada in Family Medicine. and be eligible for licensure in the province of Alberta.
