The Community Health Advocate (CHA) provides outreach to patients in the community through home visits, participation in community health events, and phone contact. Interaction between the CHA and the patient is designed to promote positive behavior change and to encourage participation in relevant health-related programs. Performs standard health screening to include but not limited to blood pressure, body mass index, cholesterol/glucose, basic medical history, and review of prescribed medications. Provides approved health education materials. Responsible for documentation and data collection to support community outreach initiatives.

Education
High School Diploma or Equivalent Required
Other Graduate of a Medical Assistant Program, Nursing Assistant Program, or equivalent Required

Experience
1 year of clinical experience in a healthcare setting Required
1 year experience in community health outreach or home healthcare Preferred
General Experience and knowledge of protocols with Electronic Medical Records (EPIC) Preferred

Licenses
Registered Medical Assistant Preferred or
Certified Medical Assistant Preferred or
Cert Clin Medical Assistant Preferred or
Certified Nursing Assistant Preferred
