Job Summary:
The Center Manager for the Jordan Valley Senior Care program has the responsibility for a wide range of disciplines and services provided through the Jordan Valley Senior Care Center, including all administrative responsibility related to direct operation of the Jordan Valley Senior Care Center. The Center Manager coordinates overall operations of the Jordan Valley Senior Care Center and serves as the liaison between direct care staff and administration.
Key Responsibilities:
- Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and initial and period assessments.
- Supervisory responsibility for RNs, rehabilitation therapists, clerical staff, social workers, transportation staff, recreational staff, dietary staff, and center aides.
- Facilitate and lead morning meetings of the interdisciplinary team (IDT); facilitate and lead regularly scheduled Intake and Assessment meetings to review participant cases on a regular basis.
- Oversee the quality of care and safety of Jordan Valley Senior Care participants, both at the Jordan Valley Senior Care center and in the community.
- Serves as a liaison between disciplines and between administration and direct staff.
- Works closely with the medical care team as part of the IDT to develop and implement participants’ individualized care plans.
- Coordinates 24- hour care delivery.
- Coordinates with the marketing department to hold “friendly visits” for new candidates at the Jordan Valley Senior Care center.
- Serves as a representative of Jordan Valley Senior Care to the community.
- Coordinates activities related to new participants.
- Manages the center’s budget; communicates regularly with Jordan Valley Senior Care administration on center operations and needs.
- Oversees overall documentation cohesiveness and accuracy in each participant’s medical record.
- Other projects and duties as assigned.
- Work independently and as an effective member of a team.
- Lead and facilitate IDT decision making.
- Trouble shoot efficiently to maintain quality operation of the Jordan Valley Senior Care center.
- Be organized within busy center and clinic environments.
- Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers and the general public.
- Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds.
- Prepare concise and clear reports.
- Identify pertinent policy and operational issues; develop strategies, solutions and recommendations.
- Multi-task in projects and their respective activities, timelines and issues.
- Effectively utilize computer and software tools.
- Only act within the scope of his/her authority to practice.

Requirements
Required Qualifications:
- Bachelor’s degree in a Health-related field required, Master’s preferred.
- At least 3 years’ experience in a supervisory capacity within a healthcare setting.
- Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the Jordan Valley Senior Care on working with a frail or elderly population upon hiring.
- Candidate must be comfortable working in a dynamic environment and comfortable working with both direct care staff and administration staff.
- Experience in successfully managing and completing projects is required.
- Experience with senior care clinical programs similar to Jordan Valley Senior Care preferred.
- Must excel in organizational and supervisory skills, with an ability to effectively handle difficult situations.
- Experience working with clinical charts and records.
- BLS certified.
