Jefferson Community Health & Life is looking for someone to work in Gardenside LTC as the Assistant Director of Nursing.
Hours: Full-time: Mainly Monday thru Friday 8am - 5pm
Some evenings and weekends as needed.
POSITION SUMMARY: The Assistant Director of Nursing (ADON) supports the Director of Nursing (DON) in the leadership, coordination, and supervision of nursing services in the long-term care facility. The ADON plays a critical role in ensuring the delivery of safe, high-quality, person-centered care in accordance with facility policies and state and federal regulations.
In addition to assisting with nursing leadership and staff oversight, the ADON has a primary responsibility in coordinating the Minimum Data Set (MDS) process, contributing to the development and monitoring of resident care plans, and supporting clinical quality outcomes. This position also provides hands-on resident care and serves as a resource, coach, and role model for the nursing team.
QUALIFICATIONS:
- Current licensure as a Licensed Nurse in the State of Nebraska RN or LPN, RN preferred
- Experience with LTC MDS comprehensive assessments and care planning preferred.
- Minimum of 2 years’ experience in LTC preferred.
- Experience with Medicare preferred.
- Basic Life Support Certification
- Must be able to wear medical grade respirator (N95/PAPR).
Leadership and Supervision
- Assists the DON with supervision of nursing staff, including RNs, LPNs, CNAs, and MAs.
- Provides guidance, support, and role modeling for staff, promoting teamwork and accountability.
- Anticipates staffing needs, helps assign staff appropriately, and addresses performance concerns in collaboration with the DON.
- Provides training, mentoring, and coaching to ensure quality care and professional growth.
- Responds to resident/family concerns with timely interventions and communicates follow-up to appropriate departments.
Resident Care
- Administers resident care utilizing the nursing process in a proficient and professional manner consistent with current standards of practice.
- Reviews, discusses, and implements physician orders appropriately; takes appropriate measures when questions arise.
- Transcribes and notes orders per facility standards; communicates orders to other departments as appropriate; schedules follow-ups as directed.
- Provides assessments of resident conditions as needed, documenting findings and updating care plans accordingly.
- Responds promptly to call lights and resident needs.
- Recognizes significant changes in resident condition, establishes priorities, and takes appropriate action.
- Serves as a resident advocate, ensuring dignity, respect, and individualized attention.
- Demonstrates expertise in the provision of Activities of Daily Living (ADLs) and delivers such care in a professional, caring manner.
Resident Safety and Clinical Quality
- Administers medications and treatments in a timely and safe manner, observing for desired and adverse effects.
- Ensures resident safety interventions are developed, implemented, and followed.
- Monitors and reports environmental safety concerns to maintain a safe care environment.
MDS Coordination
- Coordinates completion of MDS with all disciplines in accordance with regulations.
- Maintains the assessment schedule to comply with regulatory standards.
- Performs and documents nursing assessments necessary to develop an accurate MDS.
- Completes and reviews the nursing component of the MDS, ensuring accuracy and timeliness.
- Provides education, coaching, and training to care plan team and support staff to assure accurate documentation and MDS coding.
- Coordinates with therapy services to ensure appropriate documentation supports services provided.
- Monitors resident conditions to determine if a change of condition MDS is required.
Care Planning and Oversight
- Assists in the development and implementation of individualized resident care plans.
- Incorporates input from residents, families, and staff to ensure comprehensive, person-centered plans.
- Monitors effectiveness of care plans, makes modifications as needed, and ensures documentation reflects care provided.
- Contributes to and supports interdisciplinary care planning meetings.
Regulatory and Compliance
- Assists the DON in ensuring compliance with state and federal regulations, including Long Term Care Regulations
- Adheres to facility policies, safety standards, infection control practices, and compliance program requirements.
- Participates in audits, surveys, and quality improvement initiatives.
