The Palliative Care Specialist a team member of a strong interdisciplinary approach to patient care. The person in this position will function in a team designed to help the medically complex and/or chronically ill patients and family members make informed decisions. In collaboration with the medical staff and nursing staff, the Palliative Care Specialist monitors the clinical care of patients and provides support to improve patient care and patient outcomes. Incorporated within each of these functions, the Palliative Care Specialist is a professional role model and patient advocate. This person is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, and crisis intervention.
Responsibilities
Practices in a variety of primary and palliative care settings. Identifies commonly occurring episodic and chronic health problems of patient. Works interdependently with other health professionals to assist in the coordination of care to patients who have incurable, progressive disease or chronic conditions. Develops implements and evaluates management plans. Teaches and provides support for patients and families. Conducts education programs and in-services to team members (as appropriate or assigned) across the continuum of care to increase knowledge and competence of palliative care. This could be a separate section if appropriate. Conducts psychosocial assessment of palliative care patients and completes appropriate documentation. Knowledgeable of and assists with facilitation of advance directives, when appropriate..Assist in other duties as assigned to support the operational needs of the department and organization. May be required to remain on campus immediately before, during, and after severe weather and/or disasters.
