We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a LVN/Direct Care Supervisor. In this role, you will be responsible for providing high-quality nursing care to patients while overseeing direct care staff to ensure the delivery of exceptional services. The ideal candidate will possess strong leadership skills, a commitment to patient care, and the ability to work collaboratively within a healthcare team. 


Fulltime position available:

  • Monday to Friday 7am to 3pm 


As the LVN/Direct Care Supervisor, your responsibilities may include but are not limited to the below:

  • Responsible for the day to day operations of the facility 
  • Manage and supervise a team of mental health paraprofessionals
  • Provide nursing services to the crisis respite program including but not limited to:  Physical assessments, timely documentation, as well as teaching and assisting adults with mental illnesses.
  • Must be on duty 40 hours per week and must be available by telephone after hours and on weekends.
  • Correctly filling out MARS forms on a continual basis and being in constant contact with doctors, pharmacists, caseworkers, etc. with concern to the care of the residents.
  • Maintains Universal Precaution Guidelines and safety measures according to Agency policies and procedures.
  • Keeping track of all doctor’s appointments and follow-ups, including prescriptions, notes, orders, etc. given by a doctor or pharmacist as it pertains to the residents.
  •  Train and mentor new staff members, fostering a positive and supportive work environment.
  • Assists in the orientation and training of new personnel in medication management to include supervision of self-observation of medication by the residents of their individual medications.
  • Responsible for assuring all residents are treated with dignity and respect.
  • Responsible for ensuring the safety and well-being of all residents at all times.
  • Handles client health information at a minimum necessary to document data, access and carry out responsibilities of the position, maintains client confidentiality.
  • Working hand in hand with the Behavioral Health Authority on all issues and concerns of the residents.
  • Immediately report all abuse, neglect or exploitation and medication errors to The Wood Group’s Client Rights Officer and facility administrator.
  • Be familiar with The Wood Group’s policy and procedures to include employee handbook.

Requirements

 

  • Strong leadership and team management skills

Education and Qualifications:

  • Is currently licensed to practice as a vocational nurse (LVN) in Texas. Must be at least 18 years of age, must have a current Texas driver’s license.

 

Physical Demands and Working Conditions:

  • Mobility and manual dexterity to work in a standard office environment and use standard office equipment.  Must be able to bend, stoop, kneel, crouch, squat, lift 20 pounds, sit for an extended period, and reach overhead.
  • Travel which will include transporting consumers and/or picking up their medications.
  • Work may involve at-risk clients or those who pose a threat to themselves or others.
