Multispecialty, extremely busy Level 1 Trauma Center with 567 beds is seeking a Geriatric Medicine Physician within Surgery and Trauma Services.
The Geriatric Medicine Consultant for Surgery and Trauma Services is responsible for the coordination of the care for geriatric patients, age 65 or older, admitted to the trauma or general surgery services, requiring medical and multidisciplinary care. This person will facilitate acute management of patient care from admission to discharge. Using their expertise in handling the complexities of care for older adults, the Geriatric Medicine Consultant will partner with the Surgery and Trauma Attending physicians to design care plans for elderly patients to optimize their outcomes to include decreased LOS, improved home discharge disposition, and reduction in readmissions. The physician in this role will be expected to foster a strong sense of teamwork among the full breadth of healthcare providers in the acute environment including but not limited to: advance practice professionals, pharmacists, therapists, etc.
As the population ages, the demand for specialized medical professionals who are adept at addressing the multifaceted health concerns of older adults continues to rise. Geriatric Physicians, play a vital role in enhancing the quality of life for seniors by providing comprehensive care that encompasses preventive medicine, chronic disease management, and coordination of complex treatment plans tailored to the unique needs of the elderly. The Geriatric Medicine Consultant who joins our team will bring this needed perspective to support trauma and general surgery patients who are 65 and older.
Geriatric Physician Responsibilities & Duties
- Conduct comprehensive geriatric assessments to evaluate the overall health status of elderly patients admitted to the Trauma and General Surgery services
- Diagnose and manage chronic conditions commonly found in older adults, such as diabetes, hypertension, and dementia
- Develop personalized care plans that address the unique medical, psychological, and social needs of each patient
- Prescribe and monitor medications, considering the potential interactions and side effects specific to older adults
- Collaborate with Pharmacist and admitting team to review medicines and make recommendations for changes when appropriate
- Coordinate with other healthcare providers, including nurses, therapists, and social workers, to deliver holistic patient care
