JOB SUMMARY
The primary responsibility of the MDS Coordinator is to oversee and facilitate resident
assessments.
JOB DUTIES/RESPONSIBILITIES
The following job responsibilities will comply with the ADA and any/all applicable state and
federal laws. Duties and responsibilities may change or new ones may be assigned at any time
with or without notice.
- Responsible for understanding and consistently following the principles of the Happy to Oblige (H to O) program.
- Participate in and conduct in-service programs of the facility.
- Participate in facility committees as required.
MDS Coordinator job responsibilities:
- Coordinate and ensure completion of MDS, RAPs, and ancillary assessments from each department.
- Coordinate with therapy to ensure appropriate care planning (ie. restorative rehabilitation and skilled therapy).
- Coordinate family appointments.
- Schedule and oversee Medicaid meetings.
- Complete Medicare assessment schedules.
- Update census and condition sheets of residents.
- Complete the RUGs communication forms and fax to central office as needed.
- Audit charts for appropriate documentation supporting MDS and care plan.
- Assist and train new personnel with regards to care plan and MDS.
- Assist state surveyors during their visits.
- Update kardex/faces sheet through care conference.
- Coordinate overall development and implementation of the resident care plan.
- Assist with infection control program of the facility.
- Participate in Admissions Process (i.e. RUGs analysis)
- Conduct care plan conference.
ADON job responsibilities:
- Prepare, implement, and evaluate resident assessment and comprehensive care plan regularly.
- Provide for emotional and physical comfort and safety of residents.
- Observe, record, report, treat symptoms and reactions of residents.
- Follow rehabilitation plans.
- Interpret facility's policies and programs to staff.
- Evaluate, plan, and organize nursing care according to established policies.
- Consult with director of nursing on admission, room assignment, and discharge of residents.
- Assure that physician's orders are carried out.
- Supervise nursing activities and promote improvement in nursing care.
- Coordinate nursing service with other departments.
- Maintain accurate reports and records according to policies and procedures set forth; evaluate and make recommendations for improvements for welfare of residents.
- Maintain comfortable and pleasant environment and promote elimination of hazards.
- Ensure supplies are available in proper quantities, equipment is available and in proper condition, and personnel using such supplies and equipment know location and proper care of equipment.
- Receive reports from charge nurses on each shift and work to ensure maximum total care.
- Give daily report on conditions in nursing service to Director of Nursing.
- Responsible for assuring accurate recording of medications and narcotics according to procedures outlined in manual.
- Participate in nursing care as needed; responsible for direct handling of emergency situations.
- Interview and make hiring decisions for core staff on unit with input from the Director of Nursing.
- Complete performance appraisals for unit staff in a timely fashion.
- Initiate and complete disciplinary actions in partnership with Assistant Administrator and/or Director of Nursing following Heritage policy.
- Assist in hiring and termination of employees following Jennings policy.
- Coordinate and participate in staffing decisions for unit.
- Lead and direct all levels of staff on the unit.
- Plays integral role in enhancing quality indicator outcomes on unit.
- Supervise meals.
WORK ENVIRONMENT
- Office, nursing stations, drug room, resident rooms, and other areas in the facility and on the property.
- Visits to other health-care facilities and offices may be necessary.
- Occasional exposure to chemical compounds of medication and treatments.
- Occasional exposure to blood/body fluids as required for invasive treatments and procedure.
- Occasional exposure to loud and unpleasant noises due to unpredictable behaviors of residents.
- Occasional exposure to high humidity due to resident's option to control their own environment
QUALIFICATIONS
EDUCATION/EXPERIENCE:
- Must be a licensed nurse in the State of Illinois.
- Prior MDS/Care Plan experience preferred.
- Required attendance of in-services, online training, and any other education required to maintain license.
- 2-4 years of nursing experience, preferably in long-term care with leadership and management experience.
KNOWLEDGE/SKILLS/ABILITIES
- Familiarity with State and Federal regulations governing Long Term Care.
- Effective communication, organizational, and critical thinking skills are required.
- Enjoy working with older people, have a caring attitude and sincere interest in others; alert to attitudes and feelings of residents.
- Computer literacy is a must.
PHYSICAL DEMANDS
- Occasionally reaches above shoulders, crouches, kneels and balances.
- Frequently bends/stoops, squats, pushes/pulls, lifts and carriers up to 11-25 pounds.
- Continuously lifts and carries up to 10 pounds.
- Lifts and carries light objects such as resident charts without assistance.
- Occasionally transfers up to 75 pounds with assistance while lifting, positioning, and transferring residents.
- Pushes/pulls medication carts requiring force of 10-30 pounds.
- Pushes/pulls residents weighing 75-250 pounds in wheel chairs and geri-chairs.
- Stands and walks up to 75% of work day.
- Stoops, bends and squats as required to reach lower drawers in carts and file.
- Clear speech, reading and hearing skills are required continuously to comprehend and communicate with residents and co-workers, to follow written plans of care and respond to emergency alarms.
- Comprehend oral and written instructions.
- Requires ability to read small print in typed and written materials.
- Ability to read and write in English.
- Concentrates on moderate detail with some interruptions necessary to perform variety of tasks.
- Memory capable of remembering task/assignment ranging from a few hours to long periods of time.
- Ability to remain calm under stress.
- Possess good eye-hand coordination.
