PRIMARY FUNCTION

The Nurse Practitioner (NP), with additional educational preparation and experience, provides clinical consultation, formulates the plan of care, and collaborates with the interprofessional team to support patients receiving Medical Assistance in Dying (MAiD) and/or accessing KHSC Transitional Care programs.

 

The NP must know and practice in accordance with the standards relevant to the practice area as well as the legislation and regulations applicable to an NP practice:

 

  • Regulated Health Professions Act, 1991;
  • Nursing Act, 1991;
  • Laboratory and Specimen Collection Centre Licensing Act;
  • Healing Arts Radiation Protection Act, 1990;
  • Public Hospitals Act, 1990;
  • Health Protection and Promotion Act, 1990.

 

Reporting to the Program Manager, the NP works in an extended nursing role demonstrating a high level of autonomy and expert skill in order to formulate clinical decisions, appropriately manage acute illness and promote wellness within clinical neurosurgery.

 

The NP explicitly consults the inter-professional team where care is required outside of the NP scope of practice and/or not covered by a Medical Directive.

 

Within this role, the NP is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy and must demonstrate an awareness of and be responsible for actively promoting and supporting patient-and-family-centered care and engagement.

 

 

RESPONSIBILITIES & DUTIES INCLUDE:

Clinical Domain:

  • With specialized training and authorization, the NP provides consultation to patients requiring a MAiD eligibility assessment or an independent secondary assessment directed by federal law, provincial/college standards, and KHSC policy.
  • Collaborates and consults with Ethics/Legal/Risk Spiritual Health, KHSC@Home coordinators, point of care staff and Palliative Care as appropriate.
  • Completes the MAiD consenting process after confirming that all eligibility criteria and safeguards have been met.
  • Communicate effectively with patients/families about process, timing, and supports; ensures culturally safe, trauma-informed care.
  • Prescribes and administers medications and/or other treatments to support the MAiD process.
  • Plans and coordinates logistics for MAiD intervention (location, team, medications, equipment).
  • Documents each MAiD consultation and follow-up care in accordance with organizational policy, professional standards and legislative requirements.
  • Responsible for completing all MAiD legal reporting requirements and liases with the Coroner where required.
  • Consults on patients receiving care from KHSC@HOME using advanced assessment skills to develop optimal plans of care. Respond to escalations and collaborates with the Discharge Call Team to address order discrepancies, provide order clarification, and to develop follow-up plans of care for patients.

 

Leadership:

  • Is an expert in MAiD by remaining up to date on legislation changes and Canadian Association of MAiD Assessors and Providers.
  • Identifies and collaborates with MAiD assessors and providers locally, provincially and nationally.
  • Participates in MAiD continuing education opportunities.
  • Oversees coordination of the MAiD provider group to ensure appropriate coverage for assessments.

 

Education:

  • Educates/prepares patients/caregivers on MAiD eligibility, safeguards, capacity/consent, alternatives (palliative/symptom management), and what to expect after death.
  • Provides mentorship to on-boarding MAiD assessors and supports competency sign-off prior to engaging in autonomous practice.
  • Offers formal and ad hoc education to members of the interprofessional team on MAiD policies, standards and legislation.
  • Leads revision and rollout of the MAiD policy and develops tools/resources to support staff.

 

Research/Quality Improvement:

  • Monitor KPIs (e.g., referral-to-assessment time, assessments, # provisions, documentation completeness, patient/family experience).
  • Track metrics: medication discrepancies resolved, time-to-intervention, ED diversion, readmission reduction; identify themes for system fixes.

 

The proportion of time spent in the various domains of practice will be identified by the program and the population it serves.

BASIC QUALIFICATIONS:

  • Current registration with the College of Nurses of Ontario as Registered Nurses in the Extended Class with one of the following specialty classes: Adult, Pediatric, Neonatal, or Primary Health Care.
  • MAiD training (e.g., Canadian MAiD Curriculum via CAMAP) in progress or completed; submit certificate of completion when available.
  • Minimum 3–5 years NP practice in acute medicine, internal medicine, palliative/end-of-life, geriatrics, or related settings (MAiD experience preferred) OR
  • 5+ years RN experience in acute medicine plus recent NP role with demonstrated competence in prescribing and transition-of-care workflows.
  • Membership with a professional organization such as CNA, RNAO or NPAO.
  • Each NP maintains and continually improves his/her competence by participating in the College of Nurses of Ontario’s Quality Assurance requirements.
  • Advanced clinical assessment, diagnostic reasoning, and prescribing within NP scope; proficiency in capacity & consent evaluation for MAiD.
  • Exceptional communication skills for patient/family education (trauma-informed, culturally safe) and interprofessional collaboration.
  • Demonstrated strength in medication reconciliation, discharge planning, and rapid clinical problem-solving to reduce ED visits/readmissions.
  • Knowledge of MAiD legislation/safeguards, assessor independence, chain-of-custody, and Health Canada reporting.
  • EMR proficiency (Oracle Cerner) secure messaging (Teams), and comfort with data/reporting tools (forms, dashboards, basic analysis).
  • Demonstrated ability to educate staff, patients and families.
  • Demonstrated leadership skills within the interprofessional care team.
  • Excellent interpersonal, communication, presentation and documentation skills.
  • Demonstrated ability to work both independently and collaboratively with all team members.
  • Evidence of strong mentoring, teaching, team building, change leadership, organizational and prioritization skills, with the ability to work well in stressful situations.
  • Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care.
