SUMMARY: Within this role, the Diabetes Awareness Program Support (DAPS) Specialist will be assigned to work on a grant(s) that address diabetes or other cardiometabolic diseases. The DAPS Specialist provides a higher level of assistance in the development of processes and systems, including policies and standard operating procedures.  As part of an interdisciplinary team, this position works closely with our Diabetes Awareness Program (DAP) team members and will develop relationships with clients and use tools from Special Diabetes Program for Indians (SDPI) as well as other methods learned appropriate to our region and culture to promote health education and lifestyle changes to manage or prevent diabetes and other cardiometabolic diseases. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  Other duties may be assigned.

Level I:

  • Assists in the planning and implementation of diabetes program related functions, activities, classes, screenings, and workshops in the community and organization.
  • Contributes to the facilitation and flow of care and treatment of clients living with diabetes, such as teaching clients how to use a glucometer or uploading continuous glucose monitoring (CGM) readings to health information technology sources.
  • Maintains and completes a detailed monthly report of program activities accomplished in connection with grant/project. Involves coordinating and tracking monthly activity reports of appointments/classes with team members and report results to Health Disparities and Disease Prevention (HDDP) Manager.  
  • Supports ongoing follow-up with clients, as needed, in consultation with HDDP Manager, Health Care Providers, and Diabetes Nurse Educators.
  • Attends conferences, trainings, staff meetings and other assigned meetings. 
  • Collaborates with team to develop appropriate, culturally sensitive materials for client education and program outreach. 
  • Assists in process of nutrition/activity fee support opportunities provided by the Diabetes Awareness Program. Includes handling payment requests, contacting outside vendors, tracking spending, and following up with clients.
  • Demonstrates willingness to learn and gain skills to improve program effectiveness by actively seeking and utilizing available resources, such as the SDPI website.
  • Works with Diabetes Awareness Program team to ensure adherence to funding objectives including but not limited to outreach, education session implementation, curriculum development, wellness activities, data collection, client education, and reporting; provides general information/demonstration on diabetes prevention, care and management to clients, staff and community as requested. Demonstrations may include physical activity and healthy cooking.
  • Supports the Diabetes Awareness Program operations by serving as a point of contact for Diabetes Awareness Program for clients and staff. Works with Diabetes Educators to ensure that program related client appointments are secured in a timely manner. Provides support to clients in accessing program resources.
  •  The DAPS Specialist may need to attend or supervise physical activities in the community such as swimming groups, summer camp activities, or other similar activities.
  • Completes grant activities as assigned, while adhering to project timelines. This may include but is not limited to developing/creating articles, flyers, educational packets, educational social media posts, and public service announcements for distribution.
  • Maintains Electronic Health Record Diabetes Registry with data collection to meet the SDPI program outcome measures and objectives. This includes establishing a method to obtain and track data for SDPI Best Practice target groups and diabetes care completion rates.
  • Participates in the conceptualization, design, and development of all project protocols, procedures, educational and data collection activities, and infrastructure systems for successful implementation and evaluation of the project according to grant guidelines, Tribal requirements, and local cultural values.
  • Uses a variety of communication and teaching strategies, approaches, and methods that incorporate theories and concepts of adult learning, readiness for change, empowerment, motivational interviewing, and local cultural values.
  • Maintains files for program and grant documentation to verify completed activities, i.e., evaluations, consents, sign-in sheets, photos and photo releases, pre/post tests, releases of liability, applications, etc. 
  • Assists in the development of departmental policy, standard operating procedures, and quality improvement activities within the clinic as directed.
  • Works with team to maintain tracking and ordering of medical supplies.
  • Travels to other UIHS sites and locations in the community when needed.
  • Adheres to accreditation, Health Insurance Portability and Accountability Act (HIPAA), compliance standards/guidelines, and UIHS Policies and Standard Operating Procedures (SOP).
  • Maintains a professional, organized, and clean working environment.
  • Must be willing to learn more about the American Indian culture and attend at least two (2) Cultural Presentation/Training per year.
  • Must be willing to work with American Indian/Alaska Native people and be sensitive to their culture.
  • Performs all duties in accordance with UIHS Purpose, Vision, and Guiding Principles.

 

Level II:

Able to act independently in completing the above duties and:

  • Works with the HDDP Manager in data collection for grant proposals and applications for program sustainability. 
  • Maintains a level of professional development through continuing education, quality improvement initiatives and sharing of knowledge.
  • Receives education/certifications that enables ability to provide extensive education, in a specified area, in program’s health promoting classes and presentations.
  • Maintains Electronic Health Record Diabetes Registry with data collection to meet the SDPI program outcome measures and objectives. This includes establishing a method to obtain and track data and generate statistical reports to monitor and improve overall programs and collect and analyze data and information for program planning and reporting.
  • Assists tribes, community, and others with the development and adoption of policies and implementation of best practices.
  • Works with HDDP Manager in developing policies and standard operating procedures, establishing new protocol and implementation of best practices on diabetes and other cardiometabolic diseases.
  • Oversees the plans, coordination, implementation and evaluation of program activities, meetings, events, presentations, and trainings.
  • Assists in overseeing specific grant related components, including but not limited to adherence to Notice of Award conditions, quarterly and annual reporting, quality improvement and communication with funders.
  • Assists the HDDP Manager in researching and preparing documents needed to apply for new grants, which includes: the project narrative and timeline, scope of work, logic model, and other required documents.
  • Undertakes special projects assigned by the Health Disparities and Disease Prevention Manager.

 

SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.

EDUCATION/EXPERIENCE: (Degree’s must be from a US Accredited School)

Level I: Associate degree (A.A.) from two-year College or technical school and two (2) years of experience; or bachelor’s degree in an unrelated field or four (4) years related experience and/or training; or equivalent combination of education and experience.

 

Level II:  Associate degree (A.A.) and three (3) years of related experience; or bachelor’s degree in a related field such as: health education or other health field, Native American studies, communication, marketing, sociology, psychology, etc., or five (5) years of related experience or equivalent combination of education and experience.
