Join our dedicated team at Hospice of the Piedmont as a full-time LPN who, under the supervision of a registered nurse, delivers quality patient care and comfort measures, emphasizing physical, spiritual, and emotional needs.

This position will primarily work in Augusta County, VA. Candidates residing in that area are preferred.
Responsibilities/General Duties
- Deliver nursing services per Hospice policies
- Document care using EMR
- Assist physicians/RNs with procedures
- Administer medications/treatments as prescribed
- Observe, record, and report patient conditions to RN
- Follow Hospice Plan of Care for patient/family services
- Report changes in patient needs to RN case manager/team leader
- Explain Hospice services to patients and families
- Communicate with Interdisciplinary Team for coordinated care
- Participate in QA programs and required meetings
- Engage in professional development and maintain affiliations
- Maintain patient confidentiality
Qualifications
- Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
- Two years’ relevant nursing experience required
- Ability to manage all aspects of terminal care, health team service coordination, and documentation
- Basic to intermediate proficiency in word processing software

- Physical and emotional health are necessary for the accomplishment of position duties.
