We are seeking a passionate and qualified Co-Occurring Specialist to join our Assertive Community Treatment (ACT) Program—a recovery-oriented mental health service model that supports individuals with serious mental illness and substance use disorders who have not benefited from traditional outpatient programs. Services are delivered both onsite and in the community, making this a dynamic and impactful role.

What You Bring
- Master’s degree in Social Work, Psychology, or Counseling.
- PA CAADC (Certified Advanced Alcohol and Drug Counselor)
- Applicants willing and able to obtain certification within six months will be considered
- Experience working with individuals with severe and persistent mental illness and substance use disorders.
- Proficiency in electronic health record documentation.
- Ability to work within a dynamic, multi-disciplinary team.
- Prior ACT program experience preferred.
- Valid driver’s license and ability to provide community-based services.
- Capability to meet physical requirements of the service contract, with or without accommodation.
Your Role
- Conduct comprehensive assessments for individuals admitted to the ACT program.
- Deliver Integrated Dual Disorder Treatment (IDDT) to address substance use needs.
- Provide therapy and coordinate treatment interventions for assigned individuals.
- Develop, implement, evaluate, and revise treatment goals and plans.
- Offer individual support and symptom management, adjusting plans as needed.
- Collaborate with community agencies, hospitals, and other facilities for case management.
- Participate in on-call crisis intervention during nights and weekends.
- Document progress and maintain records in the electronic health record system.
