Registered Nurse Case Manager

Job Summary:

This position is responsible for overseeing, coordinating care and providing training to person served and support staff for those in our residential programs. While ensuring training, care plans and medications are updated to date and non-medical staff are proficient providing needed medical care to those served in our School/Day Treatment, and Adult Day Program.

Duties and Responsibilities:

  • Uphold the mission, values, policies and procedures of Roundup Fellowship.
  • Act as a liaison with community medical facilities, hospitals, psychiatrist, families, physicians, and other external entities in regard to the medical needs of those served.
  • Oversee medical care for those in services.
  • Coordinate care to support with person remaining as independent as possible.
  • Monitor medications and health issues of individuals served by Roundup Fellowship and consult with Program Directors, Program Managers, Medical Coordinators, and other staff regarding specific needs as well as general medical issues.
  • Monitor and obtain medical documentation such as annual physicals/physician orders etc. to ensure compliance with regulatory agencies and Roundup Fellowship’s policies and procedures.
  • Train staff and providers to become Qualified Medication Administration Personnel (QMAP).
  • Oversee the administration of medication as needed.
  • Review Medication Administration Records (MARS) for those served monitor/obtain physician orders.
  • Develop, implement, and train staff/providers on medical protocols, health care plans (which includes delegated tasks) and review and revised as needed.
  • Assist person in services in the supported decision-making process regarding their healthcare needs.
  • Train staff and provider on other medical topics to ensure needs of person in service are met while in services.
  • Transcribe verbal physician orders and enter documents into Electric Record Management (EMR system.
  • Participate in the admissions and discharge process and obtain or provide needed documentation.
  • Ensure effective communication between program, family/guardians, individuals served, and physicians where required.
  • Oversee the vision/hearing screening and immunizations for student(s) in the school.
  • Reviews and ensures training and implementation of Policies and Procedures.
  • Other duties as assigned.

Qualifications


Required Knowledge, Skills, and Abilities:

•    Must be 21 years of age or older at time of hire.
•    Bachelor’s degree in Nursing.
•    A valid Registered Nurse license to practice in the State of Colorado. Must maintain an active Department of Regulatory Agencies (DORA) license.
•    Two or more years nursing experience.
•    Knowledge and experience with intellectual/developmentally disabilities preferred.
•    Must have 5 years of driving experience and a valid Colorado Driver’s License or be willing to obtain Valid Colorado Driver’s License within 30 days of hire.
•    Good decision-making, time management and communication skills, and be responsible, mature, and flexible.
•    Knowledge of the governing rules and regulations such as Medicaid, Health Care Policy and Finance (HCPF), Colorado Department of Public Health and Environment (CDPHE), Colorado Department of Human Services (CDHS) Rules Regulating Child Care.
•    Satisfactory background checks.
•    Requires scheduling flexibility.

Physical and Mental Demands:
•    Ability to be physically present at job locations (Denver and Colorado Springs).
