$28.84- $39.34/hr DOE
Monday - Friday 8:30am-5pm (Option of working 4/10's)
Comprehensive Life Resources is looking for a qualified LPN to join our PACT team.

Scope:
The Program for Assertive Community Treatment is an evidence-based program, which provides wrap around services for individuals with serious mental illness. PACT is comprised of Mental Health Professionals, Case Managers, Certified Peer, Substance Use Professional, Nursing, and Psychiatry.
CLR’s PACT Team, focuses on client driven care, and the increasing quality of life of individuals experiencing psychotic disorders.
PACT Registered Nurses are responsible for the following:
- Collaboratively working with PACT’s psychiatric providers in ensuring psychiatric prescription orders are filled and delivered in a timely manner.
- Manages IM injection schedule and administer injections both in the community, and in the office.
- Conducts regular screenings for medical conditions, medication side effects, and monitoring existing medical conditions
- Have regular contact with mental health facilities, hospitals, adult family homes, and other providers for care coordination.
- May accompany clients to their medical appointments.
- Provide support to PACT staff through effectively communicating with clients regarding their medication. Triaging questions by staff and clients regarding psychiatric medication prior to involving psychiatric providers.
- Attend and participate in daily team meetings
Attend mandatory University of Washington fidelity-based training
Responsibilities:
- Provide nursing and medical services to clients
- Provide education regarding prescribed treatment
- Document in the progress notes the provision of care and the clinical and behavioral observations of clients
- Administer and document medications, adhering to state and program guidelines
- Provide first aid and emergency medical care as needed, following program guidelines
- Transcribe provider orders, following program protocols
- Measure and document client vital signs
- Other duties within the scope of practice as outlined by the Washington State Board of Nursing
- Attend all mandatory meetings
- Maintain licensure, certification and other requirements for the position.
- Observe and assess clients in terms of therapeutic response to medication and the nursing care plan.
- Assist clients with activities of daily living as appropriate
- Assist in monitoring and documenting medical supplies, floor stock, and pharmacy communication.
- Assist in all aspects of the laboratory specimen, collection and throughput.
- Adhere to program’s infection control policy and protocols.
- Plan and lead educational and information groups for clients as assigned.
- Work to ensure effective communication among team members during and between shifts.
- Coordinate with school and staff regarding outside medical appointments and activities.
- Transport and attend medical appointments with residents
- Work with team members to create and contribute to a positive learning environment for all.
Qualifications
Qualifications:
- Valid WA State Licensed Practical Nurse, or multi-state or compact licensure
- One year work experience preferred
- Experience or knowledge of psychotropic medications
- Current CPR certification preferred (can be completed upon hire)
- If role requires driving, must have valid driver’s license, proof of insurance and working vehicle.
