Mendota Health is a leading provider of mobile wound care services. We specialize in helping patients heal their wounds by providing the best possible treatment options in the comfort of their own home. This is an excellent opportunity for a Physician Assistant or Nurse Practitioner to join a growing organization whose mission is to transform the delivery of wound care treatment and increase access to high-quality wound care.
This position is a full-time salaried position with the flexibility to work Monday through Friday. No on-call! No mandatory weekends, evenings, or holidays!
Daily local travel is required to the patients' homes. All mileage is reimbursed. Equipment, medical supplies, and training will be provided.
Qualifications
- Graduate of an accredited nurse practitioner or physician assistant program
- A clinical background in adult, family, geriatric, podiatry, or vascular specialties
- At least one year as an advanced practice provider
- Board Certified/Prepared through AANP, ANCC, or NCCPA
- Wound care experience
- Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain
- Current, unrestricted license in the state of assignment or the ability to obtain a license
- Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority, post-where applicable
Practice Details
- The PA or NP is an experienced member of the health care team who provides advanced wound care to patients in their home setting.
- Assesses health status for wound care patient populations using advanced practice knowledge and skills.
- Develops and implements an individualized patient care plan, including coordination of care between visits.
- Thorough Assessment: Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation.
- Advanced treatments such as surgical debridement or CAMP.
- Medication Management: Order necessary medications and provide clear instructions for their use.
- Educate and Communicate: Educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams as required.
- Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
- Proven ability to function at the highest level within the scope of practice.
- Patient education and proactive disease management experience.
- Demonstrates the ability to provide care or service by adjusting approaches to reflect the developmental level of the population being served.
- This individual must demonstrate a commitment to providing exemplary patient care.
