POSITION OVERVIEW
The OCS IS117 Case Manager, in concert with the Community School Director and School Leadership Ensures that Mental Health services are provided to the Community. In Supervision with MH supervisors Responsible for timely and appropriate clinical support resources and referrals. Provide guidance, for assigned students and families to support their success in providing client care services.
KEY ESSENTIAL FUNCTIONS
- Promotes highly integrated and effective care, a one-CII culture, and adherence to agency values.
- Maintains working relationships in the community to bring awareness and education to promote the agency’s services and provides consultation to representatives of other agencies by providing regular reports as needed.
- Ensures confidentiality and maintenance of client records in compliance with HIPAA, Policies and Procedures and licensure regulations.
- Maintains knowledge of program standards and policies as developed by the agency, licensing and other governing bodies; develops personal and professional knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in in-service programs.
- Provides direct services as Case management supports attendance outreach, parent and student resources as well as build school community
- Create, maintain as well as integrate external partnerships into the school community
- Mental Health Supervisor and CSD Program Director
- Follow-up with students and with referral organizations regarding student contact and progress with referral organization
- Develop attendance improvement strategies in consultation with school personnel to assure that attendance services are in line with those projected in the school
- Obtain student attendance information for each student daily in order to initiate the tracking and documentation of each student in cohort
- Inform CSD (Director) DOE Counselor of students in need of referral services
- Provide all required information for daily/weekly/monthly/quarterly/semester/annual/academic year reports
- Support the Parent Coordinator, CSD and leadership in marketing materials for the program
- Reach out and market the program to the families/community in order to recruit students
- Support the Program Planning of School events (i.e family events, leadership activities)
- Participate in meetings/conferences with parents, School Liaison, School-based Teams/personnel and teachers for continuous dialogue communication about student’s attendance
- Input student attendance data into automated database
- Through Partnership and Networking, Outreach to outside organizations regarding such matters as education, healthcare, housing, legal issues, etc. as resources for students
- Conduct home visits
- Assist with community events including planning, preparation and execution
REQUIREMENTS
- High School Diploma or GED required.
- Associate Degree and Bachelor’s degree preferred.
- Minimum of six (6) years of experience in case management.
- Must obtain Mandated reporter (2 hours) training/certificate required.

- Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate within three (3) months of hire date.
