About the Role

ExperCARE is seeking a Licensed Practical Nurse (LPN) to join our dynamic team. The LPN plays a key role in delivering high-quality nursing care, with a primary focus on geriatric patients in both long-term care and home health settings. This position involves monitoring vital signs, administering medications, and providing compassionate, individualized support.

The LPN will work collaboratively with our interdisciplinary healthcare team to ensure care plans are carried out effectively and patient documentation is accurate. Flexibility and adaptability are essential, as the role involves responding to evolving patient needs in various care environments. Ultimately, this position is dedicated to improving quality of life and health outcomes through skilled and compassionate nursing care.


Responsibilities

  • Monitor and document patients’ vital signs; promptly report significant changes to supervising nurse or physician.

  • Administer prescribed medications and treatments per established protocols and care plans.

  • Provide direct patient care, including assistance with activities of daily living and mobility support.

  • Maintain accurate and timely patient care charts to ensure continuity of care.

  • Collaborate with healthcare team members to develop, implement, and evaluate individualized care plans.

  • Educate patients and families on medication adherence, preventive care, and health management.

  • Adapt care delivery to meet changing patient needs in long-term care and home health settings.

  • Comply with healthcare regulations, safety standards, and organizational policies.


Minimum Qualifications

  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment.

  • Completion of an accredited practical nursing program.

  • Experience in geriatric nursing or long-term care settings.

  • Proficiency in monitoring vital signs and providing nursing care.

  • Strong attention to detail in patient documentation.


Preferred Qualifications

  • Home health nursing experience.

  • Certification in geriatric nursing or related specialty.

  • Familiarity with electronic health record (EHR) systems.

  • Excellent communication skills for patient and family education.

  • Adaptability in dynamic care environments.
