Hope For Youth's Intensive & Supportive Case Management (ISCM) program is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM's intensive track (ICM), can accommodate the needs of up to 12 children and their families. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County.

The Intensive Case Management (ICM) Program at Hope For Youth is designed to help children and families maintain stability, prevent placement disruptions, and reduce the need for more restrictive care settings. Our caseworkers utilize evidence-based screening tools, such as the Casey Life Skills Assessment and the Child and Adolescent Strengths and Needs Screening, while working in close partnership with DSS and Child Protective Services.

Through ongoing interdisciplinary collaboration, our ICM team gathers a comprehensive understanding of each family's past and present functioning. This enables our case managers to provide thoughtful recommendations, effective advocacy, and ongoing support.


What You'll Do as an Intensive Case Manager

To succeed in this role, you'll bring dedication, flexibility, and a strong commitment to the children and families you serve. Key responsibilities include:

  • Supporting children in maintaining stability in their current living environment while working toward positive outcomes as outlined in the service contract.

  • Assessing safety and health concerns in the home and providing appropriate advocacy when needed.

  • Conducting two face-to-face contacts per week (home or community visits) and completing all required reports, documentation, and forms.

  • Managing a caseload of up to 4 clients to ensure individualized attention and care.

  • Participating in staff meetings, committees, supervision, and Interdisciplinary Treatment Team Meetings.

  • Engaging in ongoing training and professional development.

  • Performing additional duties as needed to support clients, the program, and Hope For Youth's mission.


A Typical Week May Include:

  • Field work (home and community visits)

  • Documentation and case notes

  • Collaboration in team meetings

  • Professional development and training opportunities

  • Responding to client needs and program priorities


Qualifications

  • Bachelor's degree in Social Work (BSW) or a related field (required)

  • Valid NYS Driver's License in good standing

  • Reliable vehicle for home visits and transportation throughout Suffolk County (mileage reimbursement provided)

  • Strong writing, documentation, and time management skills

  • Flexibility to work some evenings and occasional weekends as needed

  • Ability to respond to calls/emergencies via agency-provided mobile phone or internet technologies


Physical Requirements

  • Ability to read and write reports/log entries

  • Walking, standing, and sitting for extended periods

  • Bending, stooping, kneeling, squatting, climbing stairs

  • Repetitive wrist/finger movements (for documentation)

  • Driving regularly for client visits and transports

This position is being offered as Full Time Monday -Friday 9a-5p: flexible
