About the Department

 

A Nurse Care Manager/Sr. Social Worker position awaits you at the Forsyth County Department of Public Health. We are looking for a qualified individual to work in our Care Management for at Risk Children care management program. This individual will work with Medicaid eligible children birth to 5 and their families to improve quality or care, access to care and coordination of care. Qualified individuals must have strong assessment skills and be able to plan, implement, coordinate, monitor and evaluate plans of care that best serve the client and their family. Individuals will work collaboratively with physicians, hospital staff and other community partners.

Position Duties

A well-qualified applicant would have experience working with at risk children and would be knowledgeable of community resources.  The ideal candidate will also have the following knowledge and abilities:
 

• Thorough knowledge of the application of nursing theories, practices, principles, and techniques in the field of Public Health;
• Thorough knowledge of the current social and economic problems relating to Public Health and impacting clients;
• Ability to apply knowledge and skills needed to assess client needs and solutions;
• Ability to work independently and;
• Ability to develop and maintain effective working relationships with staff, clients, and other health care professionals.

This position has a work from home component after successful completion of a six month orientation.

Minimum Qualifications

Graduation from a four year college or university with a B.S. Degree in Nursing and one year of professional nursing experience in specialty area; or graduation from an accredited school of professional nursing and two years of professional nursing experience.

Must be licensed to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing.

A higher education level may be considered as a substitution for all or part of the experience requirement.

OR

Master's degree in social work from an accredited school of social work; 

Bachelor's degree in social work from an accredited school of social work and one year directly related experience; 

Master's degree in counseling in a human services field and one year of social work or counseling; 

Bachelor's degree in a human services field from an accredited college or university and two years directly related experience; 

Bachelor's degree from an accredited college or university and three years of directly related experience.

Valid driver's license is required.
