About the Role:

As a Physical Therapist in our Health Care Services team located in Mount Pleasant, TX, you will play a critical role in improving the physical health and quality of life of our patients. You will assess, plan, and implement individualized rehabilitation programs tailored to patients recovering from orthopedic injuries, sports-related conditions, and age-related physical challenges. Your expertise in manual therapy and orthopedic rehabilitation will be essential in facilitating patient recovery and enhancing mobility. Collaborating closely with other healthcare professionals, you will ensure comprehensive care that addresses both immediate and long-term physical therapy needs. Ultimately, your work will empower patients to regain independence, reduce pain, and improve their overall functional abilities.

As a Physical Therapist, you will:

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. 

• Applies concepts of infection control/universal precautions in performing patient care

• Assure continuity of quality patient care delivered with appropriate documentation

• Provide effective communication to patient/family, team members and other health care professionals

• Promote agency philosophy and administrative policies

• Maintain and complete all documentation within company stated guidelines. 

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license

• 1 year of home health experience preferred

• CPR certification

• Valid Driver’s license

• Valid Auto Insurance

• HomeCare HomeBase experience preferred

Physical Demands

• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Must be able to lift, carry, pull, or push up to 100 lbs..
