Position Summary
Reporting to the Program Manager the Case Manager works with homeless or precariously housed families referred to FamilyAid, to quickly and effectively stabilize and/or transition families to permanent housing. Utilizing the agency's approved two-generation (2Gen), trauma-informed care model and service plan, the Case Manager ensures that each family member accesses support and resources for daily and the longer-term supports that will allow them to maintain housing stability and break the cycle of multigenerational poverty.
- Conducts successful intakes for newly referred families.
- Assesses families' strengths and needs and develops and implements approved service plans to best meet a family’s housing and service needs, and goals defined by the agency.
- Completes housing search and application process with families.
- Assists families with accessing mainstream services benefits, income verifications and other housing-related documents.
- Collects, completes, maintains and analyzes other family data and other documentation on a timely basis that helps families to progress in their service plans and meets agency requirements and deadlines.
- Assists families through communication and/or mediation with landlords, property managers, housing agencies, and subsidy administrators.
- With assistance from 2Gen colleagues, ensures family members’ enrollment and engagement with external services focuses on financial literacy, physical and behavioral health, children’s social and emotional health.
- Conducts regular home/in-unit visits, as required by client needs, program requirements, and contract guidelines to support the family.
- Supports families in accessing and receiving onsite services and programs beyond those detailed in services plans.
Requirements
- Bachelor's degree required. BSW/MSW degree preferred.
- Professional experience in social services in a case management or social worker capacity required.
- Professional experience working with homeless, or housing insecure families preferred.
- Bilingual/Bicultural in English and Haitian Creole or Spanish preferred.
- Experience working with HMIS (Homeless Management Information Systems), specially Clarity and/or Sales Force
- Ability to work in office and field-based settings with a diverse homeless population.
- Demonstrated proficiency in data collection, analysis and utilization.
- Strong interpersonal skills and ability to work well in a collaborative, fast-paced environment.
- Experience in implementing successful client service plans.
- Experience working within a Trauma Informed Care environment strongly preferred.
- Understanding of de-escalation techniques preferred.
- Access to reliable transportation to the work location(s).
Supervision: Program Manager, Director Housing and Prevention Services, COO, and President and CEO.
Work Schedule and conditions: The position is a non-exempt, full-time role. This position is hybrid. Some work onsite at headquarters, at programs or to meet with families will be required. FamilyAid’s headquarters is located at 3815 Washington Street, Boston, MA 02130. FamilyAid’s normal business hours are Monday - Friday, 8:30 AM-5 PM, 40 hours/week, with flexibility to support clients beyond business hours, as needed.
