We are currently seeking a QMHP whose principle goal is to increase our client’s independence in all areas of their lives, and to provide them with the opportunities and resources available in the community to be independent.
The QMHP should be creative in developing strategies and support that result in people with mental illnesses achieving personal life goals. Person Centered Plans are developed based on CQLs Personal Outcome Measures and other person-centered planning tools.

Responsibilities
- Provide clinical guidance to the team about clients' mental health issues and develop and implement (or assist in the development and implementation of) clients' person-centered plans.
- Work with clients, families, natural supports, and collateral agencies in reviewing, applying, and achieving goals outlined in the person-centered plan.
- Conduct mental health assessments and recommend/implement treatment options based on these assessments.
- Conduct comprehensive psychosocial assessments (e.g., IM+CANS, LOCUS) to evaluate mental and physical health status and service needs.
- Maintain accurate and timely documentation of client interactions, progress notes, individualized service plans, quarterly reports, and discharge summaries in electronic medical records.
- Participate in staff meetings, training sessions, and professional development activities

Requirements
Requirements
- Master’s Degree in related field: counseling and guidance, rehabilitation counseling, social work, psychology, pastoral counseling, family therapy, etc.
- Minimum of one year of clinical experience
- Preferred - three years’ experience working with the Mentally Ill Population in a mental health/residential setting
License or certification:
- Valid Driver's License
- Previous QMHP certification preferred
