We offer primary care, behavioral health, substance use disorder treatment, and community support services. Duties & Responsibilities

  • Provide triage for behavioral health and support crisis and emergency situations. 
  • Deliver crisis intervention and individual counseling services; manage assigned caseload. 
  • Serve as a “bridge” for integrated care, including warm handoffs and patient check-ins with medical providers. 
  • Conduct mental health assessments and develop treatment plans. 
  • Support primary care providers, nurses, and other team members with consultation. 
  • Engage with cross-agency treatment and prevention teams in the community. 
  • Record patient case information in the EMR within 72 hours of contact. 
  • Maintain compliance with HIPAA, ethical standards, and NH state laws regarding crisis management and involuntary admittance.

     

Requirements

EDUCATION/EXPERIENCE: A bachelor's degree in: Clinical Psychology, Clinical Mental Health Counseling, Social Work, or Marriage and Family Counseling.
