Licensed Practical Nurse (LPN)

Location: Avamere at Bethany - 16360 NW Avamere Ct, Portland, OR 97229
Schedule: Full-Time, Day Shift
Wage: $35-38/hr DOE

Responsibilities/What You’ll Do:

  • Maintain accurate and timely documentation related to resident admissions, transfers, and discharges.
  • Collaborate closely with nursing and support staff to coordinate daily care plans, activities, and programs that support each resident’s overall well-being.
  • Administer medications safely and efficiently, and ensure medication records are complete, accurate, and up to date.
  • Review and update care plans regularly to reflect residents’ changing needs, ensuring personalized and goal-oriented care.
  • Help develop and implement quality care programs, policies, and procedures that align with facility standards and resident expectations.
  • Lead and support nursing staff by conducting daily rounds, ensuring high standards of care in accordance with each resident’s care plan and personal preferences.
  • Build strong relationships with residents, families, and interdisciplinary teams to promote open communication and a positive care environment.
  • Participate in state and federal regulatory inspections, ensuring full compliance and continuous quality improvement.

Qualifications/What You Bring:

  • An active, unrestricted LPN license in good standing.
  • Strong understanding of clinical practices, nursing standards, and regulatory guidelines specific to long-term care.
  • Excellent documentation skills, with the ability to chart clear, accurate, and informative nurses' notes.
  • Proficiency in Electronic Medical Records (EMR) systems and digital documentation tools.
  • Compassion, professionalism, and a genuine desire to provide exceptional resident care.
