Mountain Valley Hospice

Mountain Valley Hospice is seeking an Patient Access Team Leader to lead the process from the first patient contact through admission. This role ensures that patients, families, and healthcare partners experience a seamless, timely, and compassionate transition into hospice care.

What You’ll Do

  • Lead and coordinate the full access process, from initial inquiry through admission.
  • Support and guide Referral Intake Specialists while providing oversight for the clinical and liaison functions that support admissions.
  • Serve as the point of escalation for complex referrals, insurance authorizations, and admission challenges.
  • Partner with clinical and administrative leaders to streamline workflows and improve patient access.
  • Monitor team performance, documentation accuracy, and referral outcomes; provide feedback, coaching, and accountability.
  • Manage scheduling and ensure adequate coverage, including participation in the on-call rotation as needed.

What We’re Looking For

  • Minimum of 3 years’ experience in hospice, healthcare coordination, or physician office setting.
  • Prior leadership or supervisory experience strongly preferred.
  • Strong knowledge of referral and admissions processes, medical terminology, and insurance/authorization procedures.
  • Proficiency with EMR systems and understanding of HIPAA compliance.
  • Excellent organizational, communication, and problem-solving skills.
  • Compassionate, patient-focused, and detail-oriented.
