We consider it a privilege to make a meaningful difference, and we love what we do.  As the Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:  


  • Direct service responsibilities include supporting clients in shelter setting, including but not limited to telephone hotline assistance; information and referral services; advocacy; counseling; and community education/outreach activities.
  • Receive calls during office hours for shelter screenings and client questions.
  • Complete intake and discharge process for all clients.
  • Assess needed supports and services.
  • Identify available resources.
  • Provide referrals and linkages to other community agencies as appropriate.
  • Support clients in implementing their plans.


Case Manager Minimum Salary: $ 20.00/hour

  

Case Manager Maximum Salary: $ 20.00/hour

  

Case Manager Schedule: Monday - Friday: 8:30AM to 4:00PM

  

Qualifications:  

  • Associates Degree in Human Services or related field
  • 1 - 3 years of experience working with a diverse population
  • Valid driver’s license, proof of current car insurance and registration and access to a reliable, working vehicle
