What You’ll Do:
As a Case Manager, you’ll be a vital advocate and resource for individuals with intellectual disabilities, helping them navigate services, access supports, and achieve personal goals. Your responsibilities will include:
- Developing and maintaining individualized service plans
- Coordinating care across medical, behavioral, and social service providers
- Monitoring progress and ensuring quality of services
- Building strong relationships with individuals, families, and community partners
- Promoting independence, inclusion, and dignity in every interaction
What We’re Looking For:
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field (required)
- Experience working with individuals with intellectual and developmental disabilities (preferred)
- Strong communication, organization, and problem-solving skills
- Compassionate, patient, and committed to person-centered care
- Valid driver’s license and reliable transportation
