The Peer Outreach Specialist under the SAMHSA SUP/ Homeless services program is responsible for conducting street based outreach at encampments and at locations where PEH reside or congregate; works in tandem with Harm Reduction Program staff, with street medicine teams, and onsite at Avalon drop-in center; provides brief intake to determine immediate needs; provides health education; identifies PEH who may be interested in accessing case management, Housing, and treatment services; conducts referrals and linkages/warm hand offs to PSS and CC; Provides peer support.

RESPONSIBILITIES
- Initiate outreach to target population meeting PEH where they are at encampments and locations were PEH congregate.
- Establish and develop working relationships with clients.
- Provide comprehensive education and information through street outreach, health fairs, school relationships, community events, and other approved activities.
- Develop and maintain communication with Street Medicine and Harm Reduction Program staff on client needs.
- Provide brief intake to determine client immediate needs and identifying PEH who may be interested in accessing case management, housing, and treatment services.
- Conduct referrals to appropriate internal services and linkages/warm hand-offs to Peer Support Specialist and Care Coordinator.
- Attend regularly scheduled and impromptu meetings and maintain communication with program team members and supervisor.
- Assist in the production of required weekly, monthly, and/or quarterly data collection reports.
- Maintain outreach activity calendars and logs according to program standards.
- Assist with program evaluations and updates to assigned program and services.
- Attend appropriate community resource meetings and trainings, as assigned.
- Work in collaboration with other departments when required.
- Create, edit, and reproduce outreach materials.
- Conduct classes, training, and meetings in the community, as requested; and
- Other duties may be assigned or may be modified as business needs dictate.
QUALIFICATIONS
Education & Experience
- High school diploma or GED required; Bachelor degree in related field preferred.
- Two years’ related experience within human services and/or public health.
- Two years’ experience conducting community and/or street outreach, specifically to hard- to-reach populations.
- Experience in community organizing.
- Experience working in unhoused individuals.
- Lived experience in homelessness and/or SUD/COD preferred.
- Bilingual English/Spanish greatly preferred.
