About the Department
The Counselor III/Project Coordinator in the (AWO) A Way Out program will provide the following support services:
- Increases knowledge, awareness, efficiency, participation, and support for the program county-wide.
- Works with the research team to track participation and grant-related outcomes.
- Evaluating, assessing, and placing clients in treatment.
- Plans and implements a treatment plan with clients assigned to the program.
- Conducts case management, outreach, education, monitoring, referral/linkage, follow-ups, and documentation.
- Coordinates services team members provide. Monitors case management provided to each client.
- Monday- Friday 8:30am - 4:30pm.
- Occasional evening and weekend hours for outreach events. Job title: Counselor III Project Coordinator A Way Out
Position Duties
- Lead strategic planning for A Way Out program, including increasing awareness, participation, and performance county-wide.
- Educate community entities and individuals on AWO program.
- Manage collaboration and coordination of partners participating, including providing regular updates to key constituents. Monitors participating treatment locations and capacity.
- Track intake, screening, and treatment initiation data AWO participants. Work with the evaluation partner to compile and report data.
- Supports individuals participating in AWO program, including managing financial resources available for participant support. Assists clients in transitioning from inpatient to outpatient services.
- Provide direct services to clients, including referral, linkage, and case management, and consult with the supervisor regarding transfer/placement of a client to another level of service, as needed.
- Assist clients in obtaining public entitlements, housing, community resources, etc.
- Attend staffing with Court Services, schools, agencies, and others, as necessary.
- Organize and participate in treatment-related clinical conferences and staffing on assigned clients.
- Conduct crisis intervention/evaluation/referral to persons who present themselves at the program.
- Provide education and direction to clients, family members, and/or significant others.
- Assist in facilitating the transportation of clients.
