Position Summary:
PMHCC Case Management is a unit dedicated to providing recovery support services to individuals as they journey through their recovery process from the disease of addiction and other life challenges with the goal of achieving and maintaining long-term recovery. As case managers assist individuals in their recovery process, individuals are empowered to identify their strengths and talents and focus on the areas of need that he or she is willing to address at specific points in time. This collaborative approach is built upon the idea that all individuals, regardless of their challenges or backgrounds, are people first.
The Case Management Team Leader supports the supervisor in mentoring case managers and fulfilling administrative responsibilities as needed. The Team Leader also provides case management services to individuals who have a substance use disorder or may have a co-occurring disorder (mental illness and addiction). They support participants and promote a drug-free and sober life-style by connecting them to a range of community resources, mentoring and advocacy. Persons seeking self-sufficiency and recovery from substance abuse and co-occurring disorders are provided ongoing support and ancillary services such as: engagement, evaluation of the client’s strengths and needs, service planning and goal setting, linking, monitoring, advocating and coaching.
Duties and Responsibilities:
• Maintain a caseload of 18-25 cases of various levels of need; this number fluctuates higher or lower) based on departmental need
• Obtain 85% to 100% productivity based on available service units per month
• Maintain a compliance documentation rating of 100% each month following the documentation rating matrix
• Participate in the individual’s treatment experience by closely working with the recovering person’s care manager, therapist, psychiatrist, social worker, etc.
• Serve as CM liaison with assigned service providers to provide case management assistance and technical assistance with accessing services as needed.
• Coordinate weekly schedule for recovery specialists apprentice to shadow team members as needed by Training Manager
• Bi-weekly supervision which incorporates clinical support and a thorough review of all Case Managers’ active charts, productivity review and discussion of CM needs and/or challenges
• Attend monthly group supervision with designated Senior Manager of Case Management to ensure professional development and adherence to established benchmarks.
• Attend all mandatory trainings as designated to include both clinical and leadership development and conduct trainings on the learned information to the team as delegated
• Facilitate team meetings as designated and in the absence of supervisor
• Participate in co-facilitating monthly All-Staff Meetings as designated
• Assist supervisor with collection and review of agency documentation from team members
• Provide oversight to continued housing status per team
• Create and maintain Agency Resource Guide
• Attend interagency meetings as delegated
• Participate in CM interview process as needed
• Develop a team-based protocol to collect Philadelphia Housing Authority feedback forms and to follow up with staff regarding lapses in engagement, expired Service Recovery Plans and the status of waitlist referrals.
• Participate in provider outreach meetings as delegated
• Make revisions to daily schedule based on needed support from team regarding crisis cases
• Provide follow up regarding team members who have participants that are receiving
Recovery Support and ensuring that Recovery Support is included in the participant’s current Service Recovery Plan
• Participate in the new hire on-boarding process through field shadowing and weekly check- ins throughout the new hire’s probationary period
• Perform other duties as assigned by Supervisor, Program Manager, and Director
Skills Required:
Must have good verbal/ written communication skills and work well with people; Critical decision-making skills, ability to work independently, and be flexible/ adaptive to a changing environment. Working knowledge of MS Word and Excel; at least 1 full year as an ICM at PMHCC Case Management unit; and an overall performance evaluation of above standard or outstanding.
Must display a mastery of the following case management core competencies:
• Communication Skills: Must be able to communicate effectively with all staff members across all levels of the organization and advocate on behalf of our participants to stakeholders in a respectful and thoughtful manner.
• Compassion: Must be able to display empathy and thoughtfulness when providing services to our participants and advocating on behalf of our participants.
• Time-Management and Organizational Skills: Must demonstrate a thorough understanding of case management documentation standards, meet deadlines, be punctual, and triage crisis situations when they arise.
• Problem-Solving Skills: Must be able think critically and clearly about participant challenges and exercise sound insight and judgment when offering solutions.
Education and Experience:
Bachelor’s Degree preferred with major course work in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling or Education and one year of direct client contact in a mental health and/or drug and alcohol setting, preferred. Case Managers must complete required core Case Management trainings provided by BHTEN within one year of hire. Also, complete certification trainings in: ASAM, Addictions 101, Confidentiality, CPR, and Mental Health First Aid within one year of hire.
Physical Demands:
Sitting for a minimum of 1 hour, able to travel via car or public transportation for off-site home visits and meetings. Must have a valid driver’s license when operating a company vehicle or personal vehicle during work hours and for work-related activities.
Essential Functions:
Must have the ability to physically perform the duties and able to work in the environmental conditions required of a position in this class.
