Job Summary:
Engages community health centers, clinics and health providers to recruit eligible participants for the Welcome Baby Program. Collaborates to encourage program enrollment and participation, while providing appropriate referrals. Runs program(s) with moderate budget/impact.
Job Requirements:
Education and Work Experience:
- Associate's/Technical Degree in child development, social work, psychology, human development or equivalent combination of education/related experience: Required
- Bachelor's Degree: Preferred
- Five years' technical experience: Preferred
- Three years' experience with community groups, employer groups and grant funded projects in a collaborative setting: Preferred
- Experience working with families from diverse age, cultural and ethnic backgrounds: Preferred
Licenses/Certifications:
- Child Development Associate (CDA) certification: Preferred
Facility Specific License/Certifications:
- Hospital Fire and Life Safety (HLFS): Required
Essential Functions:
- Conducts community outreach at health centers, clinics, provider offices and other social service sites to recruit and enroll eligible participants into the Welcome Baby Program. Develops relationships with key community contacts.
- Follows up with potential participants and maintains prenatal outreach logs. Conducts participant intakes, including collecting key information in a sensitive and non-urgent manner. Collaborates with appropriate personnel to provide input for program documentation, training, procedures and overall structure.
- Operates as a member of an interdisciplinary team and collaborates with program staff to provide a continuum of services. Contacts potential participants to encourage participation in the program and enrollment. Identifies appropriate resources for participants and provide referrals during the intake process, as needed.
