Position Objective
The Health Information Associate is responsible for processing the completion of medical records within the electronic medical record. The position is responsible for working with all aspects of Health Information including; assembling medical records, scanning, deficiency analysis, records release, chart completion, etc. in a manner consistent with organization and department policies and procedures. The Health Information Associate will be expected to use good judgement in minor decision-making as is related to daily tasks, but will not be expected to make major decisions relative to policies.
Essential Responsibilities
Chart Completion/Deficiency Analysis
- Review of discharged records to ensure completion
- Assign documentation deficiencies to providers as applicable
- Follow-up on completion via deficiency analysis, report any issues with compliance to the Health Information Manager and/or Director of Health Information
Scanning Documentation
- Organize documents and assign headers as appropriate to scan into the electronic medical record
- Quality review (QR) of scanned documents to ensure accuracy upon uploading into the electronic medical record
- File incoming records timely and accurately into EPIC (Electronic Medical Record)
Retrieval of Medical Records and Release Information
- Release Medical Records in accordance with state and federal laws as well as per organization policies
- Ensure the confidentiality of all release of information transactions
Required Qualifications
- Diploma in Health Information or a minimum of 1 year Health Information experience
Knowledge, Skills and Abilities
- Strong knowledge of location specific job duties
- Ability to retrieve medical records in multiple formats
