Position Overview
The Community Health Worker provides community-based care coordination and support services by building relationships and linking residents to local programs, services, and opportunities. This role will focus on identifying, engaging, and connecting community members to available health and social services and resources to address needs such as housing, food, education, employment, transportation, and more are Management. While this role may serve all community members, there is a focus on supporting Medi-Cal members. The ideal candidate will have lived experience, familiarity with Fresno and/or Madera communities, and ability to relate to people from all backgrounds.
Essential Duties + Responsibilities
Community Health Worker Services
? Conduct outreach and build trusting relationships with community members.
? Complete health and social needs assessments and develop action plans.
? Provide health education and connect individuals to health and social services and community resources.
? Document all activities and maintain accurate records.
Coordination & Resource Navigation
? Connect individuals and families, especially Medi-Cal members, to local services. (housing, food, transportation, education, workforce, and more).
? Assist with healthcare system navigation, and appointment coordination, applications, and processes.
? Partner with other organizations to make warm hand-offs and ensure follow-through.
? Identify and connect individuals to social care programs, meeting a variety of needs.
? Accompany individuals to medical appointments and support care transitions.
? Apply trauma-informed care principles and motivational interviewing techniques.
Community Engagement & Outreach
? Participate in health fairs, events and meetings.
? Conduct presentations and build community partnerships.
? Share information about UWFM programs and help identify community health needs.
Other Duties as Assigned
As UWFM expands, this role may take on new responsibilities related to emerging community support programs and/or case management.
Qualifications
Experience + Education
? 1+ year of experience in community-based work, outreach, healthcare, or social services.
? Lived experience and strong knowledge of Fresno and/or Madera County communities.
? Experience working with diverse and marginalized populations (e.g., people who are unhoused, or have behavioral health or chronic conditions) preferred.
? High school diploma or equivalent.
? Community Health Worker certification preferred or must be willing to obtain certification within 18 months of hire.
Skills & Competencies:
? Bilingual or multilingual capabilities preferred.
? Strong interpersonal skills with clear, empathetic communication.
? Basic computer skills and ability to learn electronic health record systems.
? Excellent organizational and time management skills.
? Ability to work independently and adapt to changing program requirements.
? Comfort with mobile fieldwork and flexible hours.
? Ability to be culturally sensitive and build trust and rapport with diverse community members.
? Knowledge of community resources and ability to make appropriate connections.
? Accurate record-keeping and willingness to learn enhanced documentation systems.
Other Requirements:
? Valid state driver's license with clean driving record preferred.
? Must have valid and up to date car insurance.
? Reliable transportation for community travel.
? Ability to lift up to 30 lbs and work in various community settings.
? Willingness to participate in ongoing training.
? Evenings and weekend hours will be required on occasion.

Both part-time and full-time positions. Fresno and Madera opportunities available.
