Job Summary

The Outreach Navigator provides targeted outreach to unsheltered homeless persons living on the streets, encampments or other places not meant for human habitation (e.g. parking lots, bridges, storage facilities, forest preserves, etc.). This position meets clients where they are located (parks, street corners, shelters, etc.) and establishes rapport and reduces harm by providing critical resources such as food, water, clothing, blankets, and other necessities and determines the persons’ immediate safety needs. The Street Outreach Navigator uses a trauma-informed approach, provides crisis intervention and other skills and strategies as needed for engagement.

This position assists unhoused people navigate the social service system to identify resources that will help end their homelessness. These resources may include sheltering during severe weather, transportation assistance, immediate needs (food, hygiene, etc.), applying for SSI/SSDI, obtaining vital documents, and more. The Street Outreach Navigator is responsible for identifying and connecting with community partners, such as Mission Arvada of the Rising Church, One-Small-Step Court, Regional Homeless Navigators, etc. to support their clients in their journey toward housing.

Full-Time position. Hourly Pay Rate: $24.39 - $29.26. The base salary range represents the low and high end of Family Tree’s hiring range for this position. Actual salaries will vary depending on factors including but not limited to range of experience, years of experience. The range listed is just one component of Family Tree’s total compensation package for employees.

Example Activities:

• Meet clients where they are located to provide material goods and resources, such as: encampments, parks, community organizations, bridges, parking lots, etc.

• Perform assessments through the Coordinated Entry process and make appropriate referrals into Coordinated Entry.

• Assist program participants with immediate needs (warm/cold weather items, water, RTD tickets, etc.) vital document gathering (birth certificates, social security cards, government-issues identification, etc.), community partner referrals and homelessness verifications.

• Document client interactions after each outreach event, including: client names, client needs and client program enrollments.

• Meet with community organizations and the city of Arvada to development and maintain knowledge of resources, services and opportunities for program participants.

• Assist with interviewing, assessing and determining program eligibility for people who are homeless or at risk of homelessness.

• Promptly enter documentation and information into Client files, HMIS and otherrespective tracking systems as required; maintain documentation and data needed for monthly grant reporting.

• Maintain a full caseload.

Supervisory Responsibility:

May assist with supervision of volunteers and interns

Expectations:

Adhere to all Family Tree, Inc. established policies and procedures, including those regarding timely reporting of suspected child abuse or neglect and confidentiality of client information

Participate in at least monthly Supervision (by phone or in person).

Attend weekly staff meetings, supervision meetings, on-site and off-site trainings, and other meetings as requested.

Employees are held accountable for all duties of this job.

Must have a valid Colorado driver’s license, car insurance, and acceptable motor vehicle record.

Ensure a safe work environment and follow all safety procedures.

Working Environment:

Due to various funding streams, responsibilities may include but not limited to working with participants in Permanent Supportive Housing, Rapid ReHousing, Outreach Services and Prevention Services.

Must be comfortable going to encampments and other outdoor locations that may be under-resourced, etc. This position requires visiting locations that may or may not be handicapped accessible.

Must have access to reliable transportation on a daily basis for client visits and various off-site meetings.

Due to program participants’ availability, this position schedule requires flexibility, may include afternoon and evening hours, and very occasionally, weekend hours. This is a 40-hour a week position, primarily Monday through Friday, however can allow for a flexible schedule.

Qualifications


Required Skills/Competencies:

• Relationship Management – Experience working with under-resourced, homeless and at-risk families and individuals coming from a combination of lived experience and/or professional/educational experience working with vulnerable individuals/families.

• Flexibility and Adaptability – Open to learning and able to learn quickly; navigate change well. • Strengths-Based Approaches – Ability to practice trauma informed care, which includes being culturally appropriate, strengths-based, and providing individualized case management; able to apply a 2-Gen or multi-generational model while working with households/families.

• Teamwork and Collaboration – Committed to and skilled at building and maintaining relationships; able to work effectively with various individuals, organizations, and team members.

• Crisis Management – Ability to de-escalate high stress situations using traumainformed, evidence-based techniques. Capable of making timely, sound, and ethical decisions under pressure with limited information.

Preferred Competencies:

• Resourcefulness – Knowledge of and ability to navigate Metro Denver area community resources; an understanding of voucher administration and Medicaid billing processes.

• Digital Literacy – Strong knowledge of Microsoft Suite, HMIS and Coordinated Entry systems.

• Bilingual – Fluent in both English and Spanish.
