Position Responsibilities:
With the supervision of a licensed professional, responsible for completing required assessments, and provision of individual, group and family therapy; responsible for coordinating and implementing behavioral programming associated with the treatment plan, training of direct care staff in the behavioral management program, monitoring overall course of treatment services, and convening treatment team meetings as scheduled or as needed. Responsible for development of a adolescent’s transition plan and for liaison with DJJ, ADM and/or the Office of Family Safety and Preservation.
1. Coordinate implementation of the treatment plan.
2. Involve parents and family members in treatment process, when possible.
3. Coordinate treatment with other involved agencies and departments.
4. Provide training to direct care workers in the individualized behavioral management plan.
5. Observe and document direct care staff’s implementation of each adolescent’s individualized treatment plan.
6. Complete comprehensive biopsychosocial assessments within five (5) days of youth’s arrival to the program.
7. Complete Behavioral Health Overlay Certification, including ICD-9-CM code within ten days of youth’s admission to the program.
8. Lead the Ten Day Staffing with the youth’s treatment team in order to complete a rough draft of the youth’s performance contract and identify potential risk factors to be monitored during placement.
9. Assist in the development, review and ongoing modification of youth’s individual treatment plans.
10. Participate in weekly treatment team staffing on all youth.
11. Record entries in the youth’s chronological contact record in their case management file as needed to reflect services and/or contacts with the youth, their family, and collateral contacts.
12. Meet monthly with every family of each youth in the program (via telephone if family is unable to attend) and complete progress note entries on every family session.
13. Provide formal and informal training and clinical consultation to staff as directed by the Clinical Services Director and Program Director.
14. Attend individual and group clinical supervision meetings under the guidance of the Clinical Services Director.
15. Participate in dorm staff meetings.
16. Assist in providing individual and group counseling to residents, and train dorm staff in running the same.
17. Provide crisis intervention services to residents, and assist in referring to outside treatment providers any residents in acute crisis.
18. Assist in the development of the youth’s relapse prevention plan.
19. Assist in the transitional process prior to a youth’s release from the program and participate in an exit staffing to determine aftercare needs.
20. Assist in ongoing dorm program development.
21. Perform other duties as required by the Clinical Services Director or Program Director.
22. Perform weekly file reviews for compliance with U.S. Department of Agriculture/Department of Children and Families regulations regarding proper documentation of substance abuse diagnoses and services. (Signature of the Clinical Coordinator will be required for verification of this process.)
Requirements
1. A Master’s Level Degree from an accredited University or College in a related human service field. (Exceptions to prerequisite – Approval by Director required.)
Two years experience in working with children with serious emotional disturbances or substance abuse problems
