🕒 Schedule: Temporary, grant-funded role | 10 hrs/week for an anticipated 2.5-year term

🏢 Department: Huron Valley PACE | Ypsilanti, MI

🎯 What You’ll Do in This Role:

The Grant Coordinator will be responsible for cross-departmental coordination, process management, communication, tracking, and reporting for the Exhale Grant – Family Caregiver Support and Respite Initiative. This is a Temporary part-time role (10 hours per week) funded for approximately 2.5 years through grant support. The position offers a unique opportunity to make a meaningful contribution to improving resources and make a meaningful impact on a community-focused project. 

Primary Responsibilities:

Coordinate Resources & Outreach

  • Serve as the main point of contact for identified older adults and family caregivers participating in the Exhale Grant – Family Caregiver Support and Respite Initiative.
  • Collaborate with internal departments to connect eligible individuals with available resources, respite services, and community events.

Manage Documentation & Reporting

  • Track and manage participant information for all respite initiatives and events.
  • Maintain accurate grant files, records, and databases.
  • Collect, analyze, and report metrics and program outcomes.
  • Prepare quarterly, semi-annual, and annual grant reports in partnership with department leaders and finance.

Monitor & Support Grant Compliance

  • Monitor budgets and expenditures to ensure funds are used according to grant requirements.
  • Provide administrative support to the grants team, including coordinating internal grant-related meetings.

 

What You’ll Need:


Education: High school diploma required; Associate’s or Bachelor’s degree preferred.

Experience: At least 1 year working in long-term care, healthcare, or a related setting—experience supporting older adults is a big plus!

People Skills: Friendly, approachable, and able to build strong connections with the geriatric population.

Communication: Clear, confident verbal skills and the ability to work well with individuals, families, and team members.
