Job Title: Case Manager 
 
Home Office: Vienna, IL or Anna IL. Location is dependent on office space availability. 
 
Area of Focus: Behavioral Health   
Program(s): Housing First (215).  The Housing First Program assists adults with severe mental illness who are homeless or exiting a State Operating Psychiatric Hospital in identifying and accessing housing opportunities through the provision of rental and transitional assistance and supports.
 
Case Manager Minimum Qualifications: High School Diploma /GED equivalent
 
Case Manager Immediate Supervisor: Community Transitions Director
                                                         
Case Manager Hours of Work: Full-time, 40 hours/week. Peer crisis rotation required for RSA level staff.
 
Case Manager Experience & Credential Requirements: Qualifies as a Mental Health Professional (MHP) within 18 months of hire. Previous case management experience is preferred.
 
Case Manager Special Requirements: Travels to off-site/community based locations for direct service delivery; ability to schedule evening appointments, as needed; maintain and provide proof of valid driver’s license and vehicle liability insurance; must obtain the Certified Recovery Support Specialist (CRSS) credential within 18 months year of hire; at least 21 years of age; must complete required registry clearances and criminal fingerprint background checks. 
 
Case Manager Skills & Knowledge: Knowledge of local community based resources and human service systems; basic computer skills; excellent written and oral communication skills; skill in the delivery of rehabilitative services to adults. 
 
Case Manager job Duties include, but not limited to:
  • Provides outpatient mental health case management and community support services throughout various stages of treatment with a focus on engagement, care coordination, and discharge planning.
  • Facilitates transportation services to assist clients in connecting with internal and external resources.
  • Assists individuals in accessing basic goods and resources including housing support and state and federal entitlements.
  • Provides programmatic outreach to court systems, inpatient psychiatric systems, community health organizations, and medical facilities regarding services offered.
  • Facilitates communication across agency staff and applicable programs to ensure continuity of care and timely follow-up for individuals accessing mental health programming.
  • Completes all required forms and documentation in a timely and efficient manner.
  • Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
  • Ability to screen, assess and provide interventions for consumers that are exhibiting symptoms of trauma. 
  • Follows agency policy and procedure for all reports of abuse and neglect or suspected abuse and neglect.

Salary Range: $32,932 - $45,133 (pay is based on education level, years of experience, and credentials)
