- Provides outpatient mental health case management and community support services throughout various stages of treatment with a focus on engagement, care coordination, and discharge planning.
- Facilitates transportation services to assist clients in connecting with internal and external resources.
- Assists individuals in accessing basic goods and resources including housing support and state and federal entitlements.
- Provides programmatic outreach to court systems, inpatient psychiatric systems, community health organizations, and medical facilities regarding services offered.
- Facilitates communication across agency staff and applicable programs to ensure continuity of care and timely follow-up for individuals accessing mental health programming.
- Completes all required forms and documentation in a timely and efficient manner.
- Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
- Ability to screen, assess and provide interventions for consumers that are exhibiting symptoms of trauma.
Follows agency policy and procedure for all reports of abuse and neglect or suspected abuse and neglect.
