Job Title: Resident Wellness Director 
 
Home Office: Autumn Ridge (AR)
 
Area of Focus: Community Collaboration
 
Program(s):  Supportive Living Program (SLP) helps adults age 65+ live independently by providing them with housing in an apartment-style setting that emphasizes privacy and autonomy, while also offering services that support their health and well-being and providing a little extra assistance with the activities of daily life.
 
Immediate Supervisor: Supportive Living Program Senior Director - Heather Thompson
 
Supervises: Resident Wellness Coordinator
 
Hours of Work: Full-time, 40 hours/week 
 
Resident Wellness Director Minimum Qualifications: Registered Nurse - RN; Five (5) years direct service experience with geriatric population and two (2) years management experience is preferred.
 
Resident Wellness Director Skills & Knowledge: Fundamental knowledge of geriatrics, nutrition, death & dying, client centered planning, and the medical, intellectual and social interaction of seniors; Learning theories and their application to geriatric population; Assessment, behavioral support, written and oral communication and teaching skills; Understanding of mental health and geriatric issues; program rules, USDA, Federal and State regulations; and word processing and basic computer skills.
 
Resident Wellness Director Special Requirements: Must be 21 years of age; maintains a valid driver’s license and vehicle liability insurance; maintain First Aid & CPR Trainer certification; maintains RN licensure; must complete required registry clearances and criminal fingerprint background checks; schedules evening / or weekend hours as necessary; participates in Supportive Living Program nursing on-call rotation.
 
Resident Wellness Director job duties include, but are not limited to:
  • Provides direct supervision to assigned staff including employee orientation, performance evaluations, and administration of discipline, revision of job descriptions and day-to-day work assignments and hours of work.

  • Assures that the Supportive Living Program maintains compliance with all nursing/medical applicable standards and regulations and will ensure that compliance occurs with given policies/procedures on a day to day basis, including, but not limited to the timely completion and submission of mandatory state forms.

  • Ensures that Autumn Ridge resident prospects are managed and admitted to the Supportive Living Program and facility in a timely and efficient manner. Ensures that admissions to SLP are entered into CareLogic within 7 days of admission. Submits absentee dates into CareLogic within 7 days of absence and completes discharge within 7 days of discharge.

  • Assists the Supportive Living Program Director to oversee the development and maintenance of a waiting list for potential admissions to Supportive Living.
  • Assists the Supportive Living Program Director in the planning, management, and communication of findings related to the medical and healthcare needs identified in Outcome Evaluation Reports in accordance to Administrative Rule 146 – Supportive Living Program.

  • Provides scheduling and budget over-sight for CNA/LPN staffing patterns.
  • Assists the Supportive Living Program Director in the planning, management, and communication of findings related to the medical and healthcare needs identified in Outcome Evaluation Reports in accordance to Administrative Rule 146 – Supportive Living Program.

  • Provides direct services in Supportive Living program as needed to temporarily meet increased demand for services, when staff ratios must be maintained or for regulatory compliance.
  • Listens to and responds in a timely manner to the residents served, families and POAs.  Provides follow-up and demonstrates fairness and good judgment when meeting residents’ expectations and/or those of family or staff.
  • Acts as a liaison between the Supportive Living Program and numerous State departments within HFS as its relates to the medical and healthcare needs of the residents at Autumn Ridge.
  • Coordinates the medical and healthcare components while assisting the Supportive Living Program Manager in preparation for Administrative Rule 146 program reviews and surveys.

  • Assists the Supportive Living Program Director to ensure that the facility’s internal and external physical appearance is pleasing and maintained. Completes timely preventative maintenance requests forms and submits to the Safety Director, if necessary

Salary Range: $71,870 - $78,155 (pay is based on education level, years of experience, and credentials)
