Ritter Center is currently recruiting an Lead Case Manager for our Whole Person Care Team. This full-time, non-exempt position reports to the Program Manager, Whole Person Care based in San Rafael, CA; the role operates out of our Administrative Office.

Position Description:
The Lead Case Manager is a leadership role that provides daily support to WPC staff as well as direct services to individuals and families who are extremely low-income with a variety of barriers to permanent housing which may include mental health challenges, chronic health conditions, and substance use issues. The role predominately supports the team of case managers on client services including housing support, acquiring benefits (SSI/SSDI, General Assistance, Cal Fresh, etc.), clinical guidance, and access to needed social services. The APM is expected to be available in office four days a week.
This position maintains a small caseload of 10-12 individuals or families to which it provides case management, counseling and advocacy services. This role works directly with the WPC Program Manager to support the team to provide our services based on Housing First and Harm Reduction principles, Assertive Community Treatment (ACT) guidelines and the shared case load model. The Lead will maintain fidelity with the Housing First and Harm Reduction approach, and to expand the total number of clients served.
Essential Job Functions and Responsibilities:
Lead Responsibilities
• Attend and lead daily Triage meetings to provide support and guidance to the team. Address challenging team dynamics such as conflict, differing client approaches, and ensuring all Case Manager opinions are voiced.
• Develop agendas and lead Case Conference meetings and facilitate team discussions.
• Provide guidance training to case managers on the delivery of services centered on Housing First and Harm Reduction principles and the development of client care plans.
• Attend various Ritter and community partner meetings to represent the WPC team (MASH, Marin County Coordinated Entry, Chronic Case Conference, etc.) when requested by Program Manager.
Case Manager Responsibilities
• Engage and work with chronically homeless individuals and families with severe and persistent mental illness, physical disabilities and/or a history of substance use, a history of chronic homelessness, and/or terminal illness.
· Interview clients using clinical skills to evaluate and determine the extent of social service needs for each individual client. Monitoring health issues, making appropriate medical referrals and coordinating care as needed.
• Contact public social welfare and mental health agencies to obtain and provide information
to clients.
· Function as a part of an inter-agency multidisciplinary team; making referrals to other community resources for services and coordinating services with other social services and support agencies.
• Act as an advocate for the client. Assist clients with locating appropriate housing, assisting with the move-in process, helping clients obtain needed household items and other tasks that help clients maintain a supportive housing environment. Provide mediation and advocacy for clients between landlords and surrounding community members.
• Provide assistance with daily living tasks such as budgeting, shopping and food prep as needed while assessing client’s Activities of Daily Living capabilities.
• Create comprehensive client care plans and maintain casework records, relevant
correspondence, and accurate client case notes, related records and files on a daily basis.
• Perform other related work as assigned, such as outreach to maintain client engagement, mental health assessments, screening for eligibility for possible additional needed services.
Qualifications:
• Per OSHA requirements employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being vaccinated against COVID-19 (unless applying for an exemption) per the August 5th 2021, State Public Order for Healthcare Facilities, providers and employees.
• Bachelor’s degree in Psychology, Sociology, Social Services or a similar field, and/or 4 - 5 years of equivalent experience.
• Experience coordinating grant-funded programs requiring accurate data collection and reporting, ensuring team accountability to meet grant requirements, detailed contract management, and collaborative decision-making.
• Understanding of health care systems and the needs of individuals with complex medical/behavioral health conditions and a history of homelessness.
• Familiarity with Housing First and Harm Reduction concepts.
• Excellent organizational skills and ability to multi-task and remain calm under stress.
• Creative, out-of-the-box thinking and solutions-oriented approach.
• Experience working in a social service setting; ability to be empathetic and compassionate with clients within appropriate boundaries.
• In-depth understanding of HIPAA patient privacy protections.
• Familiarity with validated clinical assessment tools (PHQ-9, GAD-7, AUDIT-C, DAST, etc.) and Vulnerability Index (VI-SPDAT).
• Experience with Motivational Interviewing, Trauma Informed Care, and Patient Centered Care.
• Experience with the Assertive Community Treatment model is preferred.
• Knowledge of Social Services agencies within Marin County and experience maintaining close working relations with partner organizations.
• Need to possess a valid CA driver’s license with an acceptable driving record.
• Experience using computers, MS Office software, and databases is required.
• Bilingual in English/Spanish is a plus but not required.
