POSITION:

 

  • Employment Job Coach

 

 

GENERAL DESCRIPTION:

 

  • To provide employment specialist services for individual placement at a variety of fully-integrated community employment sites.

 

 

ESSENTIAL RESPONSIBILITIES/DUTIES: Essential functions of the job are denoted. Other duties may also be assigned.   • Provide consumer analysis of the following core areas of strengths and barriers to employment: social skills, soft skills, cognitive functioning and work activities.

 

  • Give sound recommendations for employment planning and services.

 

  • Procure community situational assessment sites based on consumer employment goals.

 

  • Provide support at job interviews between consumers and employer.

 

  • Attend employee-training seminars at job site as needed conducting job analysis for every day worked once a consumer starts.

 

  • Develop job duty analysis including hourly/daily/weekly task schedule analysis, task breakdown (T.A.) as well as analysis of social and performance skills needed in typical work situations.

 

  • Provide the necessary support on the job to facilitate positive working relations with co-workers, management and the public.

 

  • To supervise and train clients in mobility training, independent living and vocational training skills.

 

  • To accurately document hours spent with client as well as recording detailed progress reports for all time spent with each client.

 

  • To meet with Case Manager and/or Supported Employment Manager on a weekly basis to evaluate and report on the progress of each individual.
 
  • To provide accurate billing information and progress reports by the last day of each month.

 

  • To advocate for the consumers at all times with the Regional Centers, family, care providers and employers.
  • To assist the Case Manager in the development of the Individual Habilitation Support Plan (IHSP) and implementing the goals and objectives as defined in the IHSP.

 

  • To carry out other duties as requested by the Supported Employment Manager and Director of Program.
  • To assist the Case Manager and Supported Employment Manager in meeting with, and assessing, new clients entering into the program.

 

  • To assist in the development of jobs to meet people's individual needs and wants.

 

  • Confer with the Case Manager and Supported Employment Manager as required. Collect data during training sessions. Intervene in an incident in accordance with the written supp01t plan. Train and assist other staff in training and communication programs. Design and implement in service training to enhance entire staff's effectively.

 

  • Ensure that all incidents relating to the Health and Safety of the consumers are reported to the Supported Employment Manager and documented appropriately.

 

  • Be familiar with and abide by JNCS policies and procedures.

 

  • Participate and complete the Initial Training Program (ITP) within 90 days of employment.

 

  • Participate in regular work supervision and other training development opportunities as required/identified with position.

 

  • Complete all necessary administrative paperwork specified in managing service - mileage sheets/ timecards/ SIRS in a timely matter.

 

  • Participate in JNCS staff meeting when required.

 

  • Maintain professional conduct at all times and work within the multi-disciplinary team to support all persons served towards achieving their goals.

 

  • Other duties as assigned.

 

 

MINIMUM QUALIFICATIONS: The following are tl1e minimum qualifications which an individual needs in order to successfully perform the duties and responsibilities of this position. Please note that the minimum qualifications may vary based upon the department size and/or geographic location,

 

  • Must be 18 years old.
 
  • Must have High School Diploma or GED.

 

  • Minimum 1 year experience in Human Services field or 6 months experience within JNCS.

 

KNOWLEDGE/SKILLS/ABILITIES:

 

  • Successful completion of 12 hour JNCS Job Coach Training Course required within 90 days on the job.

 

  • Must have reliable transportation.

 

  • Ability to communicate clearly, concisely and persuasively.

 

  • Ability to use Smart Phone.

 

  • Strong customer, quality and results presentation.

 

  • Ability to interact effectively at all levels and across diverse cultures.

 

  • Ability to be an effective team member.

 

  • Understands organization culture and values.

 

 

PHYSICAL DEMANDS: In general, the following physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job.   • Must be able to see, hear, speak and write clearly in order to communicate with employees and/or other customers; manual dexterity required for occasional reaching and lifting of small objects, and operating office equipment.

 

  • Travels as required meeting functions of job responsibilities.

 

  • This position may require the use of smart phones and other mobile electronic devices to perform work functions. The employee's use of company-issued devices or pre-authorized use of the employee's personal device will comply with the agency's Cell Phone Policy at all times.
