We are a non-profit health center that provides primary care medical and dental services to all regardless of insurance status. We accept patients with or without insurance and provide health care discounts based on income and family size using the federal poverty guidelines.

About CHDC

CHDC (Community Health and Dental Care, Inc.) is a quickly growing Community Health Center in Pennsylvania with locations in Pottstown, Barto, and Boyertown, providing care for all patients in our service area, regardless of insurance status or ability to pay.

Our staff put the care and well-being of our patients at the forefront of everything we do at CHDC.

Our History

Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.

About the Physician Assistant Position

Responsible for coordination & quality assurance of clinical, programmatic & operational activities including diagnosis & treatment of illness, & patient education under medical protocols. Provides professional medical services to complement the delivery of primary health care services under the supervision of a licensed Physician. Works closely with the Chief Medical Officer, Chief Clinical Officer and Operations Director.

Duties include but are not limited to:

Clinical:

  • Provides medical evaluation, treatment and consultation services to patients of the health center. 
  • Responds to emergencies and answers medical questions. 
  • Obtains and documents, or causes to be obtained and documented, appropriate chief complaint, past medical, family and social history, review of systems, examination, medications and allergies, assessment and plan for each patient. 
  • Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals. 
  • Reviews ancillary test results and coordinates notification to the patient. 
  • Provides patient education as needed. 
  • Supervises patient care by non-health center agencies such as home health care, nursing home, or hospice. 
  • Available to patients personally or through clinical staff to answer questions and relay information regarding their care. 
  • Completion of appropriate paperwork/computer documentation to comply with insurance and reimbursement guidelines. 
  • Completion of all documentation and paperwork in a timely manner. 
  • Participates in on-call schedule. 
  • Responsible for assigned SMART goals

 Medical Administrative:

  • Participates in public relations and community outreach activities, as center shall from time-to-time reasonably request.
  • Provide assistance with OSHA, emergency preparedness, infection control and regulatory compliance.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

  • Complies with accepted dress code and maintains a professional image.
  • Demonstrates reliability and trustworthiness.
  • Manages time and other resources to meet established goals within the agreed upon time frames.
  • Demonstrates flexibility in the acceptance and completion of work assignments.
  • Participates in the department’s performance improvement activities.
  • Maintains patient/employee confidentiality in the management of information.
  • Observes the Health Care System’s compliance policies.
  • Demonstrates a strong attention to detail.

Education and/or experience

  • 3-5 years’ experience in Family Practice highly preferred
  • Current RN licensure in Pennsylvania
  • Current Nurse Practitioner Certification in Pennsylvania
  • Certification as a Nurse Practitioner by a nationally accredited nursing organization.
  • DEA Number preferred.
  • BCLS required. 
  • ACLS preferred.
  • Attendance required at assigned CHDC site(s)
