Overview: JCCA is seeking a Case Planner I to join our Preventive Services Program, which provides targeted interventions to stabilize and strengthen families in crisis — particularly when children are coping with mental illness or stress. The program uses the Solution-Based Casework (SBC) model, a strengths-based approach designed to build partnerships with families and help them develop the skills to manage real-life challenges.
Key Responsibilities:
- Provide case management and supportive services to families at risk of foster care involvement
- Conduct assessments, make referrals, and monitor client progress
- Participate in interdisciplinary team meetings with clinical staff
- Maintain compliance with NYS and NYC child welfare documentation and contact requirements
- Conduct home visits in accordance with agency and ACS guidelines
- Attend case conferences, team meetings, and training sessions
- Support planning and implementation of client group activities
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field
- Strong writing and documentation skills
- Knowledge of child welfare practices and risk factors for abuse/neglect
- Ability to work effectively with culturally and professionally diverse teams
- Spanish fluency highly preferred
Hourly Range: $27.47 – $29.71 per hour
Work Environment:
Hybrid (some remote work available)
