Job Description: The Social Worker Provides information, education, support, and advocacy for residents and their families/caregivers, in a long term care settings located at Herb Bassett Home and Pineview Terrace. The incumbent works collaboratively with residents and their families to access appropriate resources. The Social Worker Reports to the Director of Care of the Herb Basset Home and under the supervision of the Nursing Unit Managers.

Education

  • Bachelor of Social Work (BSW)

Competencies

  • Case management skills
  • Collaborative skills
  • Skilled in psycho-social assessment

Knowledge and Abilities

  • Ability to facilitate case/family conferences.
  • Responsible for accurate observation/documentation of patient related info.
  • Knowledge and skill in community outreach and crisis interventions
  • Demonstrates skill in group education, individual counselling and advocacy
