Extended Day-to-Day Substitute School Nurse
Rate: $140/day, after the 20th consecutive day rate will be $170/day, retroactive to the first day in that position.
Anticipated Dates: 6-8 weeks beginning on September 11, 2025

ESSENTIAL FUNCTIONS:
1. Provides care for injuries, acute illness and long-term management of students with special care needs.
2. Provides leadership for the provision of health services.
3. Provides screenings and referrals for health conditions.
4. Provides a healthy school environment.
5. Promotes health education.
6. Serves as a leadership role for health policies and programs
7. Serves as a liaison between school personnel, family, health care professionals, and the community.
8. Maintains a student health record for every student.
9. Stores all student health records in a locked file cabinet.
10. Determines the immunization status of all students according to the PA School Immunizations Regulations.
11. Completes School Law Immunization Regulations Annual Report.
12. Collects data, compiles reports and completes School Health Annual Reimbursement Request System (SHARRS).
13. Plans, schedules, and performs mandated services according to PA School Health Code.
14. Reviews all health condition updates on students and enters pertinent health information into Student Information System.
15. Creates Individual Health Plans (IHP) as needed, based on health conditions.
16. Plans, schedules and assists the school physician with physical examinations according to the PA School Health Code.
17. Plans, schedules and assists the school dentist with dental examinations according to the PA School Health Code.
18. Refers all students with defects noted on screenings or examinations.
19. Compiles budget supply lists yearly.
20. Assess the condition and needs of all students who enter the health room during the day; maintains an accurate daily log of all students entering the health room.
21. Provides care for staffing according to licensure.
22. Follows Procedure No. 211.3 – Communicable Diseases; excludes students who are suspected of having a communicable condition; recommending appropriate follow up.
23. May provide health instruction upon request.
24. Acts as a health consultant to students and staff.
25. Secures proper forms and physician orders for the administration of prescription and nonprescription medication during school hours.
26. Stores medications in locked cabinets.
27. Provides special medical treatments according to physician orders.
28. Utilizes community agencies when necessary.
29. Attends workshops when applicable and with Board approval.
30. Acts as a liaison for adaptive physical education teacher and medical practitioner.
31. Supports memberships of professional organizations.
32. Completes accident reports for any injury requiring medical attention or causes of absence from school.
33. Actively participates in the Student Assistance Program.
34. Maintains AED within the School District and campus.
