Pay Range: $52,000 - $78,000 Annually | Based on relevant experience
Schedule: Full-time, Monday – Friday, 8:00 a.m. – 5:00 p.m.

About the Role:
Join Quincy Medical Group as an RN Case Manager - Care Ally, where you will collaborate with physicians, patients, and families to deliver high-quality care and achieve optimal outcomes for patients with complex medical needs. This role involves assessing and coordinating healthcare services across the continuum of care, reducing hospital readmissions, and ensuring patients' medical, environmental, and psychosocial needs are met. As a key advocate, you will utilize data and evidence-based practices to enhance care quality and efficiency. 

Primary Responsibilities:

• Use predictive modeling tools to identify eligible patients for the Case Management Program.
• Enroll and manage a case load of patients, completing initial assessments, developing care plans, and maintaining required documentation.
• Coordinate healthcare services with physicians and providers to ensure appropriate care levels.
• Serve as a resource for staff, physicians, and patients regarding care planning and health benefits.
• Track and report case management activities and outcomes to the UM/QA Committee or Health Plan.
• Evaluate and refine Case Management policies and procedures as needed.
• Participate in continuing education to stay current with Case Management Standards.

Qualifications:

• Valid state nursing license and CPR certification required.
• Graduate of an accredited school for Registered Nurses.
• Previous case management experience (preferred).
• Highly organized with strong communication skills and knowledge of community resources.
