POSITION SUMMARY:

This leadership role involves educating and mentoring family medicine nurse practitioner fellows, providing clinical guidance to assigned providers at SBHCs within CHN, and delivering efficient and effective care to students visiting Community Healthcare Network’s school-based health centers.

DUTIES AND RESPONSIBILITIES (specific to Clinical Manager - SBHC):

  1. Mentors school-based NPs
  2. Reviews charts for NPs assigned to SBHC
  3. Answers clinical questions for SBHC staff
  4. Teaches as part of the NP fellowship class in didactic portion covering school-based health care
  5. Meets monthly with SBHC NPs
  6. Assists with orientation of newly hired SBHC staff 

DUTIES AND RESPONSIBILITIES (specific to Family NP Fellowship Program):

  1. Function as a dedicated clinical preceptor for FNP Fellows at a designated health center (1-2 days/week). Guide Fellows through all aspects of patient care, from assessment and diagnosis to treatment planning, while modeling best practices in communication and patient-centered care.
  2. Maintain accessibility via cell phone and/or Teams to provide on-demand clinical support and answer urgent questions for FNP Fellows, ensuring they are supported throughout their clinical experience.  

DUTIES AND RESPONSIBILITIES:

Essential Functions:

  1. Reviews/evaluates a patient’s medical and psychosocial history.
  2. Performs complete comprehensive physical examination and family planning services.
  3. Conducts procedures such as Pap smears and LARCs
  4. Develops and implements clinical policies, procedures, and protocols to ensure high-quality care delivery.
  5. Manages and supervises the healthcare providers and support staff in the school-based health centers.
  6. Provides clinical guidance and support including training and mentoring to school-based staff including nurses and clinicians.
  7. Collaborates with school administrators, teachers, and other stakeholders to promote the health and well-being of students.
  8. Monitors and evaluates clinical performance metrics and outcomes to drive continuous improvement for both SBHC sites.
  9. Ensures a safe and welcoming environment for students seeking healthcare services.
  10. Stays current with trends and developments in school-based healthcare and nursing practice.
  11. Makes referrals to appropriate sources/consultants for medical and/or social problems.
  12. Participates in patient education and counseling services, especially about family planning, STDs/HIV, preventive health, and nutrition.
  13. Enters data in a timely fashion in OSCR reporting system and ensures that clinical staff is compliant with timely data entry for OSCR reporting. 
  14. Participates in DOE-sponsored in-service education and other training programs.
  15. Adheres to each and all policies and procedures.
  16. Other relevant duties as assigned by supervisor. 

EDUCATION & EXPERIENCE:

  • Active NYS license as Nurse Practitioner 
  • Master of Science in Nursing
  • Minimum of three (3) years as Nurse Practitioner, this can include fellowship experience
  • FQHC experience and/or school-based health center experience is preferred
