Job Summary:
Under the supervision of the Day Center Director, provides personal care and services according to individual care plans for participants of the PACE Organization of Rhode Island.
- As an IDT member reports regularly on personal care services provided, participants’ self-care abilities and their expressed wishes. Attends care plan meetings as requested.
- Keeps current on and follows individual participant care plans. Adheres to PORI safety requirements and participant care plans regarding use of assistive devices (e.g., gait belts, hoyer lifts, slide boards, etc.).
- Provides personal care and services including toileting, bathing/showering, eating assistance, grooming and oral care. Performs vital signs checks and weight monitoring according to PACE-RI's policy and doctor's orders.
- Provides behavioral supervision and support for participants, monitors and responds to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc.
- Responds appropriately and in a timely manner to personal alarm devices.
- Assists participants with physical activities in support of restorative care plans (e.g. walking for exercise) as assigned.
- Uses proper universal and standard precautions and COVID-19 PPE according to PACE policy and procedure.
- Follows nursing instructions as assigned.
- Maintains accurate and current participant care and service logs as required.
- Assists with meal service and clean-up of dining areas.
- Maintains order of personal care areas including but not limited to, coatroom, bath/shower rooms, personal care/linen supply areas, participant rooms, and laundry areas.
- Assists with stocking and maintaining personal care supplies for use in Day Center. Notifies Day Center Director of low inventory to avoid running out of supplies.
- Assists with Laundry duties
- Assists with implementing recreation programs in a group setting and 1: 1 as needed; Attends to personal care needs of participants during recreation activities and assists participants in participating in recreation activities as appropriate.
- Works collaboratively with other departments to ensure that the needs of participants are consistently met and that a clean, sanitary, and safe environment is maintained.
- May be required to provide in-home care including personal care and housekeeping services.
- Escorts participants to outside appointments, as necessary.
- Understands and adheres to Pace policies and procedures and PACE's Participant Rights Policy.
- Protects privacy and maintains confidentiality of sensitive participant and agency information.
- Attends staff and participant care planning meetings as requested, and department and agency in-services as required.
- Works rotating weekends and/or holidays as assigned.
- May be requested to work at other PACE-RI sites or perform other duties as assigned.

Ability to proficiently perform clinical/skilled tasks associated within scope of position
Proficiency with MS Office Suite or similar software
Education Requirement:
High School Diploma or Equivalent, Required
Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate exposure to adverse indoor and outdoor environmental conditions (including heat, humidity, dust, smoke, pet contamination, and cleaning supplies), bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards typically associated with a medical or home care setting.
Experience:
1 year of CNA experience, Preferred
1 year of experience working with a frail or elderly population, Preferred
Licensure & Certification Requirements:
Rhode Island Certified Nursing Assistant License (CNA)
Position Requirements:
CPR Certification
Driver's License & access to reliable transportation; community-based work may be required
