- High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health or
- Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with one (1) year experience preferred.
- Must have 1 year of training or experience in vocational rehabilitation and support. Prior experience may be job development, job coaching, or supported employment.
- Must have a Level One Fingerprint Clearance Card by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- TB Test clearance required at the time of hire.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
- Conduct vocational assessments to identify members’ skills, interests, and employment goals.
- Develop individualized employment plans in collaboration with the members, focusing on skill development, job readiness, and job placement.
- Provide vocational counseling, coaching, and support to members in job search strategies, resume writing, interview preparation, and workplace behavior.
- Establish partnerships with employers, vocational training programs, and community organizations to create job opportunities for clients.
- Collaborate closely with other members of the ACT team, including Rehabilitation Specialists, BHMP, and clinical team to coordinate comprehensive care and support for the members.
- Meets with the members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.
- Works with members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
- Coordinates with all parties involved in member’s treatment according to policy and procedure and clinical protocols, including but not limited to, attending inter and intra-agency staffing, communication by telephone and letter, service/support delivery etc, as indicated on the consumer’s service plan.
- Assists members and families in accessing additional needed services and obtaining entitlements, including identification of funding services.
- Responsible for transporting members to necessary services, as outlined in their service plan, as needed.
- Assumes “primary contact” responsibility for a percentage of ACT Team members to: ensure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individual
- Keep weekly member schedule for those individuals updates, and act as first contact for those members and their families.
- Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols, and meets/exceeds member engagement goals as defined by Copa Health.
- Participates in daily team meetings as well as any additional individual treatment teams or other team communication meetings as required.
- Works in collaboration with team members to assure compliance with ACT fidelity measures.
- Implement career and rehabilitation plans as directed by the team Rehabilitation Specialist and Clinical Coordinator.
